How to Upload Records to UHS Student Health

1. Log in to MyChart: https://mychart.uky.edu/MyChart/Home/
2. Select: Menu

3. Scroll and select: Questionnaires

4. Scroll and select: (UK Student) Student Health Immunization/record upload. Please only fill out this form if you are a UK student and have been instructed to upload immunizations or medical records.
5. Answer all the required questions and then upload your records.
6. A student health nurse will review your records within 2 weeks of uploading. If you are missing any requirements, you will receive a message via MyChart.
