Dr.Mehmet Zileli has born in 1956, graduated Ege Medical School in 1980, finished residency program of Ege Neurosurgery Department in 1986, became Associate Professor in 1989 and Professor of Neurosurgery in 1996. He served as the Chief of the Neurosurgery Department in Ege University, Izmir, Turkey but left the university in July 2011.
He worked as a research fellow in Erlangen, Germany with Johannes Schramm (1987-1988) with a grant from Alexander von Humboldt Foundation. He has also worked in Bonn, Germany (1991), New Mexico, USA and Phoenix, USA (1996, 1997 Prof.Edward C Banzel, Prof.Volker Sonntag) for sort term as a research fellow.

Educational Activities
Dr.Mehmet Zileli has important contributions to education and training of spine surgeons in Turkey, by organizing 15 courses on spine surgery since 1997, three cadaver courses (2003, 2005, 2010). He has also been the host of the “Travelling Fellowship of Spine Society of Europe” (2002, 2003, 2004).
He is the founder of the first spine fellowship program in Turkey that served many national (35) and international (43) fellows. (Egypt 9, Azerbaijaan 6, Pakistan 6, Indonesia 6, Mongolia 3, Iraq 2, India 2, Bulgaria 2, Kenya 2, Nigeria 1, Uzbekistan 1, Kosova 1, Germany 1, Qatar 1).

Chiari I Malformation: Is It the Result of an instability, and Should We Perform a Fusion Surgery?

The paper by Deora et al. is a good review of the current knowledge about treatment options for C... more The paper by Deora et al. is a good review of the current knowledge about treatment options for Chiari I malformation. We are not too far from the theories trying to explain the reasons of Chiari I malformation and syringomyelia. However, we realized that the obstruction of the cerebrospinal fluid pathways was the main reason of the symptoms created by these disorders. Additional basilar invagination, and atlantoaxial dislocation were making the management more complex. We used to do combined transoral decompression and posterior fixation which were full of many tricks and high complication rates. We then learned a revolutionary surgical technique presented by Goel 3 that was simplifying the management of basilar invagination by a posterior only reduction and fixation of C1 and C2 joint. This surgical technique improved the outcomes, had lower complication rates, and the need of transoral surgeries have significantly decreased. The concept of central atlantoaxial instability has recently been introduced by Goel 4 to answer the etiology of Chiari I malformation. If that concept is true, the immobilization by C1 and C2 fixation should be the surgical technique for Chiari I. That concept was well discussed by Goel 5 and Deora et al. 1 in this issue of the Neurospine by defending contradictory views. I congratulate the editorial team for creating such a nice discussion area. The question arises if we should perform a stabilization surgery in simple Chiari I malformations. The manuscript of Goel 5 urges that it is the best solution for Chiari I. However, literature is full with papers evidencing that a simple foramen magnum decompression is successful in more than 70% of the cases. There is no literature yet comparing a decompressive surgery with fixation surgery. Even if it is an unstable condition, the necessity of doing a stabilization surgery is not obvious. Deora et al. 1 have done a nice analogy with lumbar spinal stenosis. Lumbar spinal stenosis may also be considered as a result of instability of a motion segment. The degeneration process starts from the intervertebral disc, then facets degenerate and an unstable condition occurs. Disc bulging, ligament hypertrophy and osteophytes are secondary to that unstable condition. Should we always consider a fusion surgery in lumbar spinal stenosis? Simply no. Even if there is degenerative listhesis, necessity of pedicle screw fixation or other fusion techniques is not hundred percent. Central atlantoaxial instability may be the reason of Chiari malformation. However, we need biomechanical and clinical studies to investigate this theory. We also need high class clinical studies comparing the outcomes of simple decompressive surgeries and fixation surgeries.

They have compared two groups: One with anterior cervical discectomy and fusion (ACDF) surgery for C3–C4 level and the other with C5–C6 level. They have found that C3–C4 level patients to have more diffuse spondylotic changes than C5–C6 patients.

This paper aims to analyse long term results of retrogasserian RF lesions for trigeminal neuralgi... more This paper aims to analyse long term results of retrogasserian RF lesions for trigeminal neuralgia. Materials and Methods: Between 1992 and 1998 a total 213 patients presented with trigeminal neuralgia were treated. Mean age was 59±13 (min 24, max 90), Male/Female ratio was 89/121. The pain was on right in 134 cases and on left in 79 cases. The distribution of the pain in divisions was: division I: 4, division I & II: 22, division II: 52, division II & III: 68, division III: 36, all divisions: 31. Secondary trigeminal neuralgia presented in 21 cases (9,9%): The etiology of trigeminal neuralgia was attributed to multiple sclerosis in 8 (3,8%), cerebellopontin angle tumor in 5 (2,3%), vascular lesions in 4 (1,9%), blood disorders in 2 (0,9%), basilary invagination and Chiari syndrome in 1 (0,5%), arachnoid cyst in 1 (0,5%) Trigeminal neuralgia was accompanied with hemifascial spasm in 4 (1,9%), with hemifascial spasm & tinnitus in 1 (0,5%). Bilateral trigeminal neuralgia presented in 7 patients (3,3%). The technique we used was performed under fluoroscopic control and division specific lesions by radiofrequency thermocoagulation at 75 degree centigrade for 60 seconds. During RF lesions IV diprivan anesthesia was applied. Results and Discussion: We examined 116 patients with more than 5 year follow ups and total application of 142 RF rhizotomies. Initial success rate was 98%. Recurrence rate was 15%. Recurrence interval was on average 18 months (1-36) Number of RF applications in this group was one time only in 93 cases, 2 times in 20 cases, 3 times in 3 cases. Complications were transient 6th nerve palsy in 2, carotid artery puncture in 1, anesthesia dolorosa in 1. There were no death and no significant morbidity. Conclusions: Retrogasserian RF thermolysis for trigeminal neuralgia is an easy and and non-ambulatory technique which allows division specific pain relief with very high success rate and small number of reversible complications.

We aimed to investigate the contribution of the touch imprint cytology (TIC) of core needle biops... more We aimed to investigate the contribution of the touch imprint cytology (TIC) of core needle biopsies (CNBs) to the diagnosis in spinal lesions. mAterIAl and methOds: Sixty cases with spinal CNB and TIC were included in the study. Hematoxylin and Eosin (H&E) and May-Grunwald-Giemsa (MGG) stained TIC slides and CNB of each case were blindly reevaluated by two pathologists. TIC and CNB findings were categorized as follows: 1-Nondiagnostic 2-Benign nonneoplastic 3-Benign neoplastic 4-Malignant 5-Malignant lesions that can be typed. results: TIC and CNBs were nondiagnostic in 62% and 45% of all cases, respectively. Malignancy could be identified and typed in 23% of imprint preparations and 35% of CNBs. When TIC and CNB diagnosis were compared, it was seen that diagnostic categories were just the same in 70% of the cases. In 37% of the cases benign and malignant groups could be distinguished. In 20% of the cases diagnostic categories were totally different. The percentage of the nondiagnostic material in CNBs can reach up to 45%. The findings in TIC and CNBs were compatible with each other. We observed that TIC was diagnostic on its own especially in carcinoma metastasis and contributed to the assessement of cytologic detail in hematologic malignancies.
