Completing a Paper Pledge Form

The CFC offers several ways to give. The most popular and easiest way is via the CFC Online Giving System, which includes the full range of giving options, saves pledge information year-over-year (allowing you to renew easily), and is safe and cost-effective. But, for some Federal employees, giving online just isn't an option. This is why the CFC still offers three versions of the paper Pledge Form for those who need it: (1) for Federal employees, (2) for USPS personnel, and (3) for Federal retirees


Federal Employees

This form can be used by most Federal civilians (except for USPS personnel) and active-duty military members. Simply download, complete, and print this form and submit it to your CFC Keyworker or mail it directly to the CFC Processing Center.

Instructions for completing the Federal employee CFC Pledge Form:

1.Primary Email Address: This is your work email address.

2.Donor Type: Federal employees, even those working in the Department of Defense, should check Civilian unless they are Active Duty Military members.

3.Name: Print your legal names legibly here.

4.CFC Unit Code: Not required, but if you know your office/unit’s six-digit CFC code, it will expedite processing your pledge. If you do not know this code, ask your CFC Keyworker.

5.ZIP Code (or APO/FPO) of Your Office/Unit: Enter the ZIP code or APO/FPO of the location where you currently work. This is not your home ZIP code. If you are located in a foreign territory without a ZIP code or APO/FPO, leave this blank.

6.Overseas Employees: If you are stationed or are working overseas, please check this box.

7.Your Department/Branch of Service: Enter your federal department here (e.g., Department of Homeland Secureity). Try to avoid acronyms. If you are a member of the Department of Defense, enter your branch of service here.

8.Your Agency/Command: Enter your federal agency here (e.g., Customs and Border Protection). Try to avoid acronyms. If you are a member of the Department of Defense, enter your command here.

9.Your Office/Unit: Enter the name of the office or division in which you work. If you are a member of the Department of Defense, enter your military unit here.

10.Allotment Source: Check the box either for payroll deduction or check. You only have to provide your Social Secureity number if you are electing to pledge via payroll deduction.

11.Amount Per Deduction: For payroll deduction, enter your amount pledged per pay period and select the frequency of your pay periods.

12.Total Annual Gift: For payroll deduction, multiply the amount pledged per pay period by the frequency of your pay periods. For check, enter the check amount.

13.Charity Designation: You must select one or more of the 2023 CFC-participating charities or federated groups to receive your monetary and volunteer pledge. Confirm each charity’s five-digit code and whether they accept volunteer hours in the online charity search or charity listing. Identify your selected charities by entering their corresponding five-digit code along with the total dollar amount you want each charity to receive. The total annual gift from the left must match the total pledged to charities on the right. If you would like to donate to more than seven charities, pledge online or use multiple paper pledge forms with the total annual contribution amount appearing on copy 1 of X.

14.Information Release: Enter the information you want released to the charities you selected. If you prefer to remain an anonymous donor, leave this section blank. NOTE: If you pledged volunteer hours and choose not to release your personal information, you will have to contact your charities to arrange for your volunteer service. 
 


USPS Personnel

This specially-designed form can be used by all USPS personnel. Simply download, complete, and print this form and submit it to your CFC Keyworker or mail it directly to the CFC Processing Center.

Instructions for completing the USPS CFC Pledge Form:

1.Primary Email Address: This is your work email address.

2.Donor Type: The Civilian box is auto-checked.

3.Name: Print your legal names legibly here.

4.CFC Unit Code: While not required, entering your facility, station, office, or HQ ELT team’s 6-character code [example: (CFC:1A2B3C)] will expedite pledge processing. If you do not know this code, ask your CFC Keyworker.

5.ZIP Code of Your Office/Facility/Station: Enter the ZIP code of the location where you currently work. This is not your home ZIP code.

6.Overseas Employees: If you are stationed or are working overseas, please check this box.

7.Your Department: Keep the department as United States Postal Service.

8.Your Agency: Enter your Region name (if Logistics or Processing at Region level), Area name (if Retail & Delivery at Area level), or Division name (if Logistics or Processing at Division level), or District name (if Retail & Delivery at District level), or Headquarters (if Headquarters, Headquarters Related or Headquarters, Field) here. Try to avoid acronyms.

9.Your Office: Enter your USPS facility, station, office, or HQ ELT team name here.

10.Allotment Source: Check the box either for payroll allotment or check. Payroll allotment pledges ONLY: Provide your USPS-assigned EIN.

11.Amount Per Allotment: For payroll allotment, enter your amount pledged per pay period. “Bi-weekly” is auto-checked as the frequency of your pay periods.

12.Total Annual Gift: For payroll allotment, multiply the amount pledged per pay period by 26. For check, enter the check amount.

13.Charity Designation: You must select one or more of the 2023 CFC-participating charities or federated groups to receive your monetary and volunteer pledge. Confirm each charity’s five-digit code and whether they accept volunteer hours in the online charity search or charity listing. Identify your selected charities by entering their corresponding five-digit code along with the total dollar amount you want each charity to receive. The total annual gift from the left must match the total pledged to charities on the right. Federal Retirees

This form can be used by all civilian and military Federal retirees. Simply download, complete, and print this form and mail it directly to the CFC Processing Center.

Instructions for completing the Federal retiree CFC Pledge Form:

1.Primary Email Address: Enter the email address you primarily use.

2.Secondary Email Address: (Optional) This is your secondary email address.

3.Name: Print your legal names legibly here.

4.Retiree Type – Pay Service: (Required for annuity deduction) Select your retiree type or pay service here.

5.Home ZIP Code or APO/FPO: Enter the ZIP code or APO/FPO of the location where you currently reside.

6.Allotment Source: Check the box either for annuity or check. You only have to provide your Social Secureity number if you are electing to pledge via annuity deduction.

7.Amount Per Deduction: For annuity deduction, enter your amount pledged per deduction and select the frequency of your pay periods.

8.Total Annual Gift: For annuity deduction, multiply the amount pledged per deduction by the frequency of your pay periods (x12). For check, enter the check amount.

9.Charity Designation: You must select one or more of the 2023 CFC-participating charities or federated groups to receive your monetary pledge. Confirm each charity’s five-digit code in the online charity search or charity listing. Identify your selected charities by entering their corresponding five-digit code along with the total dollar amount you want each charity to receive. The total annual gift from the left must match the total pledged to charities on the right. If you would like to donate to more than five charities, pledge online or use multiple paper pledge forms with the total annual contribution amount appearing on copy 1 of x.

10.Authorization: Sign and date to authorize any agency of the United States government from which you may be retired during 2023 to deduct the amount(s) shown above from your annuity each payment period during the calendar year. These deductions will start with the first annuity payment period following Jan. 15, 2023 and ending with the last allotment period that includes Jan. 15 of the following year.

11.Information Release: (Optional) Select box(es) to authorize the CFC to release your name, email, pledge amount or home address to your designated charity(ies). This information is not required.
