Abstract:
Over the past five years activity based reimbursement using the DRG system has gradually been implemented in Denmark. Today hospitals receive between 20-50% of their revenues as activity based reimbursement. It is believed that this will increase hospital activity and productivity more than by using capped budgets supplemented by contracts, the standard practice until the turn of the millennium. One of the objectives of this article is to analyse whether the increase in productivity has been higher over the period 2002-2004 when partial activity based reimbursement was used, compared to 1996-2001 when global budgets (capped budgets) dominated reimbursement scheme. The average annual productivity increase from 1996 to 2001 was around 1%, and the period 2002-2004 does not deviate from this. The second objective is to analyse differences in productivity across counties. These analyses show that the various degrees of activity based reimbursement across counties cannot explain differences in productivity. The results also indicate that some of the differences in productivity are due to differences in hospital structure and geographical spread of the counties.
