The Student/Scholar Request for OIE Services form should only be used by returning/continuing students and scholars.

Submit the Student/Scholar Request for OIE Services (pdf) form for the following requests:
- Travel signature for a DS-2019
- DS-2019 replacement or correction
- DS-2019 for a spouse or child
- Travel letter (H-1B only)
- To request a signature on the OIE Social Security Number Form (pdf), have your on-campus hiring department sign the form by hand and then drop the completed form (including the request for services form) off to the OIE front desk.
