Amac:Women’s Health Initiative (WHI) calismasinin yayinlanmasinin takiben hormon tedavisinin (HT)... more Amac:Women’s Health Initiative (WHI) calismasinin yayinlanmasinin takiben hormon tedavisinin (HT) surdurulmesi veya kesilmesine olan etkisini incelemek. Materyal ve Metod: Calisma Temmuz 2002 ile Temmuz 2005 tarihleri arasinda 816 postmenopozal kadin ile gerceklestirildi. Hastalara sosyo-demografik ozellikleri, HT kullanim bilgileri, WHI calismasi hakkinda bilgiler soruldu. Bulgular: Kadinlarin ortalama yaslari 49,3±3,6 ve cevap oranlari %54,7 idi. Katilimcilarin %22,1'i HT almakta iken, %77,9'i HT kullanmamaktaydi. Kadinlarin universite mezunu, lise, ortaokul ve ilkokul mezunu olanlari sirasiyla 131, 99, 60, 157 seklinde idi. Ilkokul mezunlarinda HT kullanim orani %25,universite mezunlarinda %20 olmasi tedavinin egitim durumundan etkilenmedigini gostermekte idi. 2002 yilinin ikinci yarisinda HT baslama orani 18,6%, 2003'te % 30,5 seklinde olup 2004 yilinda %21,5'e, 2005 yilinda %20,9 oranina geriledi. Çalışmanın Yapıldığı Yer: Marmara Üniversitesi Tıp Fakül tesi Kadın Hastalıkları ve Doğum ABD Materyal ve Metod: Marmara Üniversitesi Kadın Hasta lıkları ve Doğum Kliniği'nde termde ağrılarıyla başvuran 60 gebe amniotomi sonrası mekonyum içeren ve içer meyenler olarak iki grupta incelenmişler, doğum sırasın da neonatal olarak ilk nefes alınmadan umbilikal korddan arteryel kan gazları alınmış ve Apgar skorları kayde dilerek postnatal takipte mekonyumlu grupla, mekonyumsuz grup arasındaki farklar ortaya konmaya çalışıl mıştır. Bulgular: Mekonyumlu ve mekonyumsuz grupta incelenen 30'ar hastanın karşılaştırılmasında; doğum öncesi kulla nılan parametreler ve doğum sonrası kordph'sı, kan gaz ları, périnatal morbidité oranları açısından her iki grup arasında anlamlı bir fark bulunamamıştır (p<0.05). Sonuç: Fetal monitorizasyon bulguları güven verici olduğu sûrece, travayda mekonyum fetusa ek bir risk getirme mektedir.

To compare the efficacy of two antiandrogens, spironolactone and flutamide, in the treatment of h... more To compare the efficacy of two antiandrogens, spironolactone and flutamide, in the treatment of hirsutism. Twenty women with idiopathic hirsutism were randomized to receive either flutamide or spironolactone. Twenty hirsute women were recruited from patients presenting to the hirsutism clinic in Marmara University, Istanbul. Each patient underwent a complete medical and gynecological examination as well as endocrine profile, hematologic, hepatic, and renal function analyses. Hirsutism scores were determined according to the Ferriman-Gallwey scoring system. These tests were then repeated at 3, 6, and 9 months of therapy. Ten patients received 250 mg of flutamide two times per day, and 10 patients received 100 mg of spironolactone for 9 months. Ferriman-Gallwey scores were decreased significantly in both groups at the end of 9 months. The percent of change in hirsutism scores in flutamide and spironolactone group were as follows: 26.4% and 20.9% at 3 months; 39.5% and 32.9% at 6 months; and 46.4% and 39.6% at 9 months, respectively. There was a trend toward a better response with flutamide that did not achieve significance. None of the hormonal parameters changed significantly during this period of time. Irregular bleeding was observed in five patients (50%) of the spironolactone group, whereas none of the patients in the flutamide group experienced menstrual irregularity. Conversely, dry skin and increased appetite were experienced by two patients (20%) in the flutamide group but not in the spironolactone group. These data suggest that both spironolactone and flutamide were similarly effective in treatment of hirsutism, and the pure antiandrogen flutamide is a safe and effective alternative in treatment.

Design: Prospective randomized single-blinded study. Setting: A tertiary hirsutism clinic. Patients: Forty-two premenopausal patients with hirsutism were selected. Interventions: Subjects were randomized to receive either 100 mg spironolactone and an oral contraceptive (OC) containing 150 p,g desogestrel and 30 p,g ethinyl E2 or 50 mg CPA daily on days 1 to 10 of the menstrual cycle, which was administered with 35 p,g ethinyl E2 daily on days 1 to 21. Main Outcome Measures: Hirsutism scores were measured according to Ferriman-Gallwey scoring system and side effects were monitored for 9 months of treatment. Blood samples were taken at each visit for assessment of endocrine, biochemical, and hematologic parameters. Results: Hirsutism scores were decreased significantly in both groups at the end of 9 months. The percent of change in hirsutism scores in CPA and spironolactone group were as follows: 19.23% ± 14.77% and 24.48% ± 14.27% at 3 months; 39.01% ± 19.77% and 37.46% ± 16.90% at 6 months; and 51.89% ± 20.87% and 46.39% ± 16.10% at 9 months, respectively. There was a trend toward a better response with CPA treatment, which did not achieve significance. None of the patients stopped treatment because of side effects. Conclusion: The present data suggest that both spironolactone and CPA were similarly effective in treatment of hirsutism.

Methods: A survey was performed on 816 postmenopausal women between July 2002 and July 2005. Questions included sociodemographic characteristics, determinants of HT, use of knowledge and source of information regarding the WHI study. Results: The average age was 49,3±3,6 years. Among the participants 22,1% were using HT whereas 77,9% were not . Of the women; 131, 99, 60, 157 were graduated from university, high school, middle and primary school respectively. HT usage was 25% among primary school and 20% among university graduates, which appeared not to be affected by educational level. The rate of starting HT was 18,6% in the second half of 2002. Background/Aim: To investigate the expression and value for diagnosis of the genes, p53 and pTEN,... more Background/Aim: To investigate the expression and value for diagnosis of the genes, p53 and pTEN, the protein, Ki-67, and the receptors, estrogen and progesterone, in differentiating smooth muscle tumors of the uterus. Material and Method: Seventeen samples of leiomyosarcoma, 2 smooth muscle tumors with uncertain malignant potential (STUMP), 9 atypical myomas and 15 leiomyomas were stained immunohistochemically. The χ2 test was used for the statistical analysis of the data. Results: The malignant side of the spectrum was strongly stained for Ki-67 and p53 while uniformly decreasing toward the benign tumors. The results were found to be statistically significant (p < 0.0001). The staining for progesterone receptor was also statistically significant, but the tumors that were considered benign, such as leiomyoma and atypical myoma, were the ones strongly stained (p = 0.005). This study is designed to compare endometrial thickness measured by transvaginal ultrasound with endometrial pathology by dilatation and curettage. Methods. Fifty-four women with postmenopausal bleeding were evaluated. Endometrial thickness, including both layers of the endometrium, was measured by transvaginal ultrasound after which fractional curettage was performed and samples taken were then dispatched for histologic examination. Results. Median endometrial thicknesses of 5 mm, 8.5 mm and 6 mm were found for benign, hyperplastic and carcinomatous endometrium, respectively. Of the eight hyperplastic samples, two cases had 4 mm, and one case had 3mm endometrial thickness. Similarly, of the nine malignant samples, three cases had an endometrial thickness of 3 mm. Three mm has been found to be the best cut off point for endometrial abnormalities in postmenopausal bleeding with 100% sensitivity but low specificity (13%). Conclusion. Fractional curettage seems to be the best method for detecting endometrial abnormalities in women with postmenopausal bleeding.
