Oral lichen planus (OLP) is a chronic immune-mediated mucocutaneous disorder predominantly in whi... more Oral lichen planus (OLP) is a chronic immune-mediated mucocutaneous disorder predominantly in white women after the fifth decade of life, rarely affecting children. Symptomatic OLP is usually treated with systemic and/or topical corticosteroids, but its prolonged use may cause several adverse effects. An eight-year-old girl presented bilateral white reticular plaques associated with atrophic areas involving the buccal and labial mucosa, and tongue dorsal surface with burning complaining. Medical history was non-contributory and an incisional biopsy was performed. Clinical and microscopic features were highly consistent with OLP diagnosis. Hence, 20 punctual low-level laser therapy (LLLT) sessions were performed, followed by significant clinical improvement and symptom discontinuation. We suggest that LLLT appears to be a successful treatment for childhood OLP, with good acceptance by pediatric patients.

T... more Multiple salivary gland tumors (MSGTs) are most common in the major than minor salivary glands. The most MSGTs are synchronous, either benign or malignant. A 61-year-old woman was referred presenting nine submucosal nodules, firm to fluctuant, being five nodules on the right side and four nodules on the left side of the upper lip. An incisional biopsy was performed. Hematoxylin and eosin staining was performed in 5-µm sections for histopathologic analysis. Immunohistochemical reactions were carried out in 3-µm sections in accordance with manufacturer's instructions. The histopathological analysis showed focal area containing low-grade polymorphous adenocarcinoma (PAC) and multiple canalicular adenomas (CAs). Immunohistochemical analysis for each lesion was carefully investigated. Here, we present an unusual case of synchronous PAC and multiple CAs of the minor salivary glands, affecting the upper lip, which appears to be the first case showing PAC and CA.

Stafne’s bone cavity (SBC) is an asymptomatic lingual bone cavity situated near the angle of the ... more Stafne’s bone cavity (SBC) is an asymptomatic lingual bone cavity situated near the angle of the mandible. The anterior variant of SBC, which shows a radiolucent unilateral ovoid lingual bone concavity in the canine-premolar mandibular region, is uncommon. A 73-year-old man was referred for assessment of loss of mandibular bone. Panoramic radiographs and computerized tomography scans showed a well-defined lingual bony defect in the anterior mandible. Analysis of imaginological documentation, made 14 years ago, revealed a progressive increase in mesiodistal diameter and intraosseous bony defect. The soft tissue obtained within the bony defect, microscopically revealed fibrous stroma containing blood vessels of varied caliber. The current anterior lingual mandibular bone defect case is probably caused by the salivary gland entrapped or pressure resorption, which can explain the SBC pathogenesis.KEYWORDSBone defect; Mandible; Cone beam computed tomography; Diagnosis; Case report.

Sua patogenese permanece desconhecida, embora comumente seja aceito que se desenvolva a partir de remanescentes do epitelio odontogenico, presentes na da mandibula, maxila e gengiva. Destaca-se que algumas dessas lesoes sao notadamente solidas, enquanto outras se mostram com aparencia cistica, sendo as primeiras (solidas) denominadas de tumor dentinogenico de celulas fantasmas.

Activin A-knockdown increased the migration of HUVECs. In addition, activin A stimulated the phosphorylation of SMAD2/3 and the expression and production of total VEGFA, significantly enhancing the expression of its pro-angiogenic isoform 121. The present findings suggest that activin A is a predictor of the prognosis of patients with OSCC, and provide evidence that activin A, in an autocrine and paracrine manner, may contribute to OSCC angiogenesis through differential expression of the isoform 121 of VEGFA.

An imaginological examination revealed superficial bone resorption. By microscopy, typical features of Gingival cyst of the adult (GCA) were observed. However, in the lesional area there are no teeth and consequently periodontium. Thus, the clinicopathological correlation favored a diagnosis of alveolar cyst of the adult, counterpart of the GCA but on the edentulous maxilla or mandible.

Epithelioid angiomatous nodule (EAN) is a rare benign vascular proliferation, regarded as part of... more Epithelioid angiomatous nodule (EAN) is a rare benign vascular proliferation, regarded as part of the morphologic spectrum of benign and malignant epithelioid vascular lesions. EAN is a rare lesion affecting the oral mucosa and, to date, only three cases have been reported in the English-language literature. We report the second EAN case affecting the gingival mucosa of a 69-year-old female patient. Oral examination revealed an asymptomatic, well-defined nodule exhibiting a smooth and erythematous surface, measuring 0.8 cm in greater diameter. The lesion was fully excised and histopathological study showed a mucosal epithelioid proliferation with solid and organoid growth patterns, and vascular lumens scattered focally throughout the lesion. The large epithelioid cells showed intracytoplasmic vacuoles and vesicular nuclei with prominent nucleoli, surrounding by scarce extravasated erythrocytes. Immunohistochemistry showed positivity for vimentin, ␣-SMA, CD34, focally for D2-40, and Ki-67 was 15%. Noteworthy, numerous immune cells (HLA-DR+/CD68+/CD163+/FXIIIa +) scattered throughout the lesion, were detected. To the best of our knowledge, this is the first report, which highlights the immune cell population, with M2-like phenotype, as an important component of EAN, suggesting the participation on their etiopathogenic mechanisms.

Describimos aquí doscasos adicionales afectando la cavidad oral, los cuales estuvieron localizados en el carrilloizquierdo. El examen histológico mostró lesiones fusocelulares bien circunscritas en padrónestoriforme y hemangiopericítico exhibiendo alternancia de áreas hipercelulares conhipocelulares. Los hallazgos inmunohistoquímicos fueron similares en ambos casos revelandofuerte inmunorreactividad para vimentina, CD34, bcl-2 y negatividad para actina músculoespecífico, actina músculo liso, S100 y citoqueratinas de amplio espectro. A 22-year-old female was referred to our hospital with a large expansile cystic lesion in the left mandibular ramus associated with a clinically visible, partially erupted third molar. A diagnosis of paradental cyst was made. After marsupialization of the lesion, histopathological analysis of the surgical specimen showed an unusual exuberant HRG reaction supported by scarce fibrous stroma. This was a case of exuberant HRG reaction that caused extensive bone destruction.

The aim of this study is to report 2 cases of melanotic neuroectodermal tumor of infancy (MNTI), emphasizing the analysis of intratumoral immune cells by immunohistochemistry. Study Design. Case 1: A 6-month-old girl presented with a 3-cm tumor in the anterior region of the left maxilla. Case 2: A 4-month-old boy presented with a 4-cm tumor in the anterior region of the left maxilla. Microscopically, case 1 had predominantly neuroblast-like cells supported by fibrillary neuropil-like stroma arranged in an alveolar pattern, whereas case 2 exhibited scattered melanocyte-like and neuroblast-like cells supported by fibrovascular stroma. A large immunohistochemical panel for characterizing intratumoral macrophage and dendritic cell subsets was performed. Results. Immunohistochemical analysis indicated positivity for HLA-DR, XIIIa, CD68, and CD163 (range 6%-50%) mainly on the fibrovascular stroma, suggesting M2 macrophage-like cell phenotype. CD138 was overexpressed in the tumor stroma. Conclusions. Results suggest the involvement of M2-polarized macrophages in the MNTI pathogenesis, which may act by modulating tumor growth and/or tumor stromal remodeling.

Here, we report the case of a 65-year-old South American woman with a submucosal nodule on her right buccal mucosa. The nodule was slightly tender and painful. Differential diagnoses included mesenchymal (lipoma or fi brolipoma, solitary fi brous tumor, and neurofi broma) or glandular benign tumors (pleomorphic adenoma) with secondary infections. We performed excisional biopsy. A histopathological examination revealed a dense fi brous capsule and a single female fi larial worm showing double uterus appearance, neural plaque, well-developed musculature and intestinal apparatus. Dirofi lariasis was diagnosed, and the patient was followed-up for 12 months without recurrence.

This denomination encloses multiple myeloma (MM), solitary bone plasmacytoma and extramedullary plasmacytoma. MM consists of a clonal proliferation of plasma cells based in the bone marrow, with various degrees of differentiation. Neoplastic cells usually produce great amounts of monoclonal light or heavy chains of immunoglobulin that can be detected in serum or urine. The disease is more frequently in men and the average age at diagnosis is about 60 years. The diagnosis is established by blood and urine exams and medullary biopsy. Patients may present renal failure, bone pain, fatigue, recurrent infections and nervous system dysfunction. Oral manifestations may be the first sign of MM, highlighting the importance of the dentist in the early diagnosis of the disease. Central ossifying fibroma is a benign slow-growing tumor of mesenchymal origin and it tends to oc... more Central ossifying fibroma is a benign slow-growing tumor of mesenchymal origin and it tends to occur in the second and third decades of life, with predilection for women and for the mandibular premolar and molar areas. Clinically, it is a large asymptomatic tumor of aggressive appearance, with possible tooth displacement. Occasionally treated by curettage enucleation, this conservative surgical excision is showing a recurrence rate extremely low. The objective of this study was to report a case of a 44-year-old woman, presenting a very large ossifying fibroma in the mandible, which was successfully treated with curettage, and to conduct a brief literature review of this lesion, focusing on the histology, clinical behavior, and management of these uncommon lesions.

The aim of this study was to assess the immunohistochemical expression of p63 protein, epidermal ... more The aim of this study was to assess the immunohistochemical expression of p63 protein, epidermal growth factor receptor (EGFR) and Notch-1 in the epithelial lining of radicular cysts (RC), dentigerous cysts (DC) and keratocystic odontogenic tumors (KOT). For this study, 35 RC, 22 DC and 17 KOT were used. The clinical and epidemiological data were collected from the patient charts filed in the Oral Pathology Laboratory, University of Ribeirão Preto, Brazil. Immunohistochemical reactions against the p63, EGFR and Notch-1 were performed in 3-µm-thick histological sections. The slides were evaluated according to the following criteria: negative: <5% of positive cells, low expression: 5-50% of positive cells, and high expression: >50% of positive cells. Moreover, the intensity of EGFR and Notch-1 expressions was also evaluated. This lesion is easily diagnosed and in some cases distinct microscopic variations such as osseous, oncocytic and squamous metaplasia may be found. These metaplastic alterations probably are associated with the lymphocytic infiltrate usually present in denture hyperplasia. We present a case of denture hyperplasia containing salivary gland tissue with ductal alterations mimicking an oral inverted ductal papilloma.
