Introducao: As modificacoes hormonais podem mostrar alteracoes do perfil lipidico nas diferentes ... more Introducao: As modificacoes hormonais podem mostrar alteracoes do perfil lipidico nas diferentes fases do ciclo menstrual, principalmente devido alteracoes nos concentracoes sericas de estrogenio. Pouco se pesquisa em nosso meio sobre o impacto dessas variacoes hormonais no perfil lipidico. Objetivo: Avaliar as alteracoes do perfil lipidico plasmatico durante as fases folicular e lutea do ciclo menstrual. Metodo: Estudo prospectivo, realizado com 10 mulheres saudaveis, media de idade 23 anos, com ciclo menstrual regular, nos ultimos seis meses ou mais. Foram realizadas coletas durante as fases folicular e lutea de cada paciente, definidas no 4o e 21o dias do ciclo, apos 12 horas de jejum. Analisou-se colesterol total e fracoes, triglicerides, glicose, TGO, TGP, creatinina, acido urico, hemograma e urina I. To relate our experience with thrombolytic therapy in 8 patients with cardiac metal prosthesis co... more To relate our experience with thrombolytic therapy in 8 patients with cardiac metal prosthesis complicated with thrombosis. Eight patients with clinical and echocardiographic diagnosis of cardiac valvar prosthesis thrombosis were treated. The treatment efficacy was evaluated by clinical and echocardiographic improvement. The authors recognized improvement in all 8 patients. One case of hemorrhage was observed (coxo-femoral articulation). Late death were observed twice: 24 months after, sudden death and 30 months later by hemorrhagic stroke. The results indicated that thrombolytic therapy is best way to treat patients with cardiac valve thrombosis, instead of surgical treatment.

To analyse clinical aspects, diagnosis and treatment of infective endocarditis (IE), in order to ... more To analyse clinical aspects, diagnosis and treatment of infective endocarditis (IE), in order to guide medical procedures and indicate the surgical treatment. 83 patients were submitted to clinical treatment of IE between January 1980 and December 1987; clinical aspects, laboratory and pathologic-anatomy findings, the site of infection and the entrance of micro-organism were studied. 37 (44.6%) patients were males. Fever was present in 75 (90.4%) cases, cardiac murmur in 76 (91.5%), splenomegaly in 28 (33.7%) and heart failure (III and IV) in 32 (39.8%) patients. Blood culture was positive in 55.5%; staphylococcus 50% of blood cultures; anemia was present in 66 cases (79.5%) and high serum mucoprotein in 58 (92%); echocardiography was 85.7% positive. The mortality was 39.76%; congestive heart failure was the main cause of death; 78.1% of these ones occurred in the first 15 days of antibiotic-therapy. We report a case of a 59-year-old patient diagnosed with chagasic cardiomyopathy, who manifested ... more We report a case of a 59-year-old patient diagnosed with chagasic cardiomyopathy, who manifested sudden heart failure while hospitalized, evolving to death due to cardiogenic and septic shock. Anatomical-pathological studies revealed infarction of the papillary muscles together with histological changes compatible with 48 to 72 hours of evolution. Pulmonary edema was considered the cause of death, probably related to mitral regurgitation of ischemic nature. The cause of the papillary muscle infarction was not elucidated by study of the coronary tree, which presented no signs of recent thrombosis. Explanation for the papillary muscle infarction in this patient may be related to the presence of alterations in microcirculation represented by vasodilation and the consequent phenomenon of "stealing" of blood flow in this territory to the detriment of other areas, or due to the fact that the papillary muscles may represent convergence zones of two distinct coronary circulations.

Purpose-To evaluate epidemiological, clinical and therapeutic aspects of elderly patients with ca... more Purpose-To evaluate epidemiological, clinical and therapeutic aspects of elderly patients with cardiovascular disease in Brazil. Methods-Elderly patients with cardiovascular disease treated in 36 centers of cardiology and geriatrics were investigated through a questionnaire applied to those who had an appointment during the analyzed period. Results-2196 elderly patients ranging from 65 to 96 years of age were analyzed, 60% of which were females. The main risk factors were: sedentarism (74%); high blood pressure (53%), high LDL-cholesterol (33%), high total cholesterol (30%), obesity (30%), low HDL-cholesterol (15%), diabetes (13%) and smoking (6%). A higher prevalence of females existed among those with > 3 risk factors. The main reason for the medical appointment was high blood pressure (48%). Stress test and coronariography were requested more often in males. The most common diagnoses were hypertension (67%), and coronary disease (29%). The most often used medications were diuretics (42%). Conclusion-There was high prevalence of risk factors (93%), mainly in females; sedentarism was the most common risk factor and prevalence increased with age; hypertension was the most common reason for a medical appointment. Diuretics were the most used drugs; congestive heart failure was the main disease associated to hospitalization (31%) and emergencies (10%).

This review discusses atrial fibrillation according to the guidelines of Brazilian Society of Car... more This review discusses atrial fibrillation according to the guidelines of Brazilian Society of Cardiac Arrhythmias and the Brazilian Cardiogeriatrics Guidelines. We stress the thromboembolic burden of atrial fibrillation and discuss how to prevent it as well as the best way to conduct cases of atrial fibrillatios in the elderly, reverting the arrhythmia to sinus rhythm, or the option of heart rate control. The new methods to treat atrial fibrillation, such as radiofrequency ablation, new oral direct thrombin inhibitors and Xa factor inhibitors, as well as new antiarrhythmic drugs, are depicted.

Hoje o conhecimento medico se duplica em menos de 5 anos! Deixamos a fase de ausculta, ECG e RX de torax e nos embrenhamos no conhecimento genetico / molecular das doencas. E importante que o medico esteja sempre atento aos avancos na medicina para trazer o melhor para o atendimento de seu paciente. Todavia, como o volume de informacoes e muito grande, as diretrizes e “guidelines” tem realizado o papel de concentrar as informacoes de modo objetivo, facilitando a leitura e incorporacao das informacoes para a pratica clinica. Recentemente tem se observado que, mesmo com o as diretrizes, o conhecimento medico tem volume de informacoes exponencial. Somente nos ultimos 3 anos a Sociedade Brasileira de Cardiologia publicou quase 20 diretrizes sobre diversos temas em cardiologia, demonstrando que a atualizacao do medico consome muito tempo em leitura. ...
Resumo: Sabendo-se dos acometimentos causados pelos transtornos mentais nas criancas, nota-se a i... more Resumo: Sabendo-se dos acometimentos causados pelos transtornos mentais nas criancas, nota-se a importância de diagnosticar e intervir previamente. Este trabalho teve como objetivo avaliar o desempenho funcional de criancas e adolescentes com transtornos mentais. Tratou-se de uma pesquisa de campo descritiva, exploratoria com abordagem quantitativa. Participaram da pesquisa 24 criancas e adolescentes usuarias do Centro de Atencao Psicossocial Infantil Juvenil da cidade de Patos, Paraiba. Os dados coletados foram obtidos atraves de um questionario social e demografico e pela Medida de Independencia Funcional. No mais, foram analisados e tabulados no Statistical Package for the Social Sciences . Os resultados demonstram que o sexo masculino e predominante em criancas e adolescentes com transtornos mentais, sendo o transtorno do espectro autista (TEA) o diagnostico mais prevalente. Six patients with Q-wave myocardial infarction in the ECG, two with coronary disease, two with me... more Six patients with Q-wave myocardial infarction in the ECG, two with coronary disease, two with metabolic alterations, one with acute myocarditis and another with ischemic stroke had an improvement of ECG tracings with disappearance of the Q wave. All had normal plasmatic levels of CPK and CKMB. It is believed that metabolic transitory disturbance of the myocardium increases the rest transmembrane potential turning the cell nonresponsive to electrical stimulus and without mechanical activity (inactive electrical zone, not a necrosis zone, which is an anatomo-pathological diagnosis).
