Student Records Forms

These forms help students manage their academic records, request official documents and make changes to their enrollment status. If you have any questions about the forms listed, contact the Office of Student Records at (630) 942-3838 or recordsoffice@cod.edu.

  • Academic Program Change Form
    Use this form to update or change your academic program. If you wish to change your major, add a new program of study, or switch between degrees or certificates, this form allows you to make those adjustments officially. This is especially useful for students who are exploring different career paths or academic interests.
  • Application for Degree or Certificate

    This form must be submitted when you are ready to apply for your degree or certificate. Once completed, the Office of Student Records will review your academic progress and confirm whether you have met the graduation requirements for your program of study.

  • Course Description Request

    Use this form to request a detailed description of any course offered at the College of DuPage. Course descriptions may be required for transfer credit evaluations, professional purposes or to assist you in planning your academic future.

  • Final Grade Report Request

    This form allows students to request a copy of their final grades for a specific semester. It’s helpful for those who need a record of their grades for personal or academic use, such as applying for scholarships, internships, or transferring to another institution.

  • Petition For Failing Grades

    If you have received failing grades in one or more courses and believe there were extenuating circumstances that affected your performance, this form allows you to formally petition for a grade change or exception. Supporting documentation may be required.

  • Prerequisite Verification Request

    This form is used to verify if you have met the prerequisites for a course you wish to enroll in. If you believe you have equivalent experience or qualifications that satisfy course prerequisites, this form will allow you to submit documentation for review.

  • Transcript Order Form

    This form allows you to request an official transcript from the College of DuPage. Transcripts are often needed when applying for graduate programs, transferring to another institution or verifying your academic history for employment or certification purposes.
