    I, Gangireddy Pedda Anji Reddy S/o.Chinna Konda Guruva Reddy,
    aged 37 years, Hindu, resident of Chandragiri Village, Mundlamuru Mandal,
    Prakasam District, do hereby solemnly affirm and sincerely state as follows:I am the deponent herein and 1st petitioner in the above O.P. The 2nd
    petitioner is my daughter and the 3rd petitioner is my son.

    I am giving this

    The above O.P is filed for

    compensation for the death of my Wife, who died in a Motor accident.


    I submit that I did not file any similar application in any court of law for
    the self same relief nor I did not entrust the same to anybody except the
    Advocates, Sri.Abburi Venkateswarlu, V.Venkata Pavan Kumar, who are filing
    the above O.P.
