    AFFIDAVIT OF SELF-ADJUDICATION

    I, ABEL D. REYES, of legal age, Filipino, single and a resident of Piapi,


    Dumaguete City, Negros Oriental; after having been duly sworn to in accordance with
    law, depose and state:

    1. That I am the husband of Ana Calo-Reyes who died without any last will and
    testament on September 23, 2012 in Dumaguete City as evidenced by his Death
    Certificate issued by the City of Dumagute hereto attached as Annex A and made an
    integral part of this Affidavit;

    2. That at the time her death, my wife had no debts, liabilities or obligations to any
    persons, agency or institution;
    3. That the properties she left are parcels of land consisting:
    A. Original Certificate Title No. 0V9 described as Lot No. 412 Dumaguete Cadastre,
    situated in Piapi, Dumaguete City with an area of Four Thousand Twenty Three (4,023)
    square meters, more or less;
    B. Transfer Certificate of Title No. 1-6997 described as Road Lot No. 4-H, being a
    portion of Road Lots. No 4 of planPsd-14107. LRC (GLRO) Cad Record No. 140,
    situated in Dumaguete City, containing asn area of Forty Four (44) square meters, more
    or less;
    C. Transfer Certificate of Title No. 1-2188 described as a Two Hundred Seventy Six
    (276) square meter area of Lot No. 410;
    D. Transfer Certificate of Title No. 2786 described as Road Lots No 3 and 5 with an area
    of Two Hundred Sixty (260) square meters more or less, and Five Hundred Thirty Nine
    (539) square meters more or less, respectively;
    E. A Bank deposit with BDO worth P50,000.00
    F. A Bank deposit in BPI worth P5,500,00.00.
    Hereto attached as Annex A to F and made an integral part of this Affidavit;

    4. That pursuant to Rule 74, Sec. 1 of the Rules of Court, I hereby adjudicate unto myself
    the above-described real estates and bank deposits by means of this Affidavit and hereby
    files the same with the Register of Deed of DumaguteCity with the request that the said
    adjudication be made effective without judicial proceeding as prescribed by the
    aforementioned provision of the Rules of Court.

    IN WITNESS WHEREOF, I have hereunto set my hand this 20th day of November
    2015 in Dumaguete City, Negros Oriental, Philippines.
