For direct IV dilute each vial with 10 mL of 0.9% NaCl for a
concentration of 4mg/mL administer over at least 2 min. For
intermittent Infusion dilute further with 100 mL of D5W,
0.9% NaCl or LR for a concentration of 0.4 mg/mL
administer over 15 min at a rate of 3mg/min
