EAGLE SCOUT RANK APPLICATION ROUTE SHEET

This route sheet is for use with the rank advancement requirements that became mandatory on April 1, 1999.

Scouts having completed any Eagle Scout rank requirements after April 1, 1999, must earn the Personal Fitness
Merit Badge and Cycling or Hiking or Swimming Merit Badges.

Scoutmasters: Complete all items in the “Troop” column.


District Board of Review: Complete all items in the “District” column.

Request Rank/Merit Badge Profile from Council Service Center
All information legible
Council number, type of unit, national unit number
Applicant’s full legal name (30-character maximum) spelled correctly and legibly
Applicant’s address: no abbreviations
Unit type, local unit number, location; no abbreviations
Date of entry in Scouting; First Class and Star rank board of review dates
At least four months between First Class and Star rank board of review dates
At least six months between Star and Life rank board of review dates
Webelos Scout and Arrow of Light questions answered
Scout’s date of birth (required)
Applicant meets age requirement criterion
At least six months between Life and Eagle board of review dates
References checked
21 merit badges earned (Day, Month, Year listed for each). On merit badges 6 and 9 cross out those not
applicable. (NOTE: 4 required for Star, 3 additional required for Life with a total of 12 required and 9 optional)
Must serve at least six months in a position of responsibility between Life and EagleRank board of review dates
and prior to 18th birthday; see application for list of positions.
Eagle service project completed between Life and Eagle rank board of review dates and prior to 18th birthday.
Statements by applicant are not to be sent to National Eagle Scout Service
Growth conference completed prior to 18th birthday.
Applicant’s signature and date on application.
Unit leader’s signature and date on application.
Unit committee chairman’s signature and date on application. (Cannot be same as unit leader’s signature)
Eagle Rank Application with signatures and dates stated above taken to Scout Service Center to be verified.
Eagle board of review signatures and date on application
Completed Advancement Report
If board of review is 3 to 6 months after applicant’s 18th birthday, submit documentation stating reason.
If board of review will be conducted more than 6 months after applicant’s 18th birthday, contact the Boy Scout
Division at the national office for procedures to follow prior to scheduling board of review.
Eagle service project report returned to applicant
Council certification signature and date on application
Scout Executive’s signature and date on application
Copy made of the completed application with signatures for permanent council file.
Application sent to the Eagle Scout Service center.
NESA application with check for $25 mailed to NESA (optional)
Credentials recorded in local council record of Eagle Scouts
