ito ay para makakuha ng mga datos na makakatulong sa aming research.
Inaanyayahan po namin kayo na sagutan ang mga tanong sa questionnaire nhg buong
puso at katotohanan para malaman naming ang inyong tunay na nararamdaman.
Ang unang grupo mga tanong ay ukol sa mga espesipikong mga tanong tungkol sa iyong
gamot at ang ikawalawang grupo naman ay ang iyong tingin sa mga doktor ang gamot sa
pangkalahatan.
Ang pag-aaral na ito ay boluntaryo. Kung gusto po ninyong hindi sumali, wala pong
problema. At kung sumang-ayon naman kayo at gusto niyo nang huminto, puwede rin po
ninyo ihinto kahit ano mang oras.
Wala pong tama at mali sa mga sagot.
Kung kayo ay papayang sumali sa pag-aaral, kailangan lang pong pumirma sa consent
