I have read the Terms and Conditions pertaining to upgrade and am aware of the eligible Credit Limit. I agree to abide by the Terms and Conditions as listed below:
I hereby accept the offer to upgrade my card to JetPrivilege HDFC Bank Diners Club Credit Card(s) and declare that the information included in this form is
true and correct. I accept that HDFC Bank is entitled in its absolute discretion to accept or reject this form. I will be bound by the Terms and Conditions as
may be in force from time to time and receipt / use of the Card shall be deemed to be acceptance of those Terms and Conditions. I understand Service Tax
as applicable from time to time will be levied on fees, interest and other charges, as per government guidelines. I agree to abide by the Terms & Conditions
of the Cardmember Agreement and to be levied various charges mentioned in the Schedule of Charges as and when applicable to me. Credit Limit on my
Card account may be reviewed as per Banks policies specified from time to time and the Bank reserves the right to revise (increase or decrease) and / or
unconditionally cancel the limit assigned on my Card. I also understand and agree that the Bank is authorised to change any or all of the Terms and
Conditions and Schedule of Charges at its own discretion with prior intimation. I agree to receive my Card statements through email on the email ID
mentioned in the form. I am aware that my new Credit Card would be automatically linked to NetBanking in case I am already a HDFC Bank NetBanking
customer. I understand that HDFC Bank will assign the Credit Limit according to the Bank's internal guidelines and reserves the right to exercise its sole
discretion to honour my transactions above the assigned Credit Limit on a case-to-case basis taking my spend and payment pattern(s) into consideration.
I understand that in case the Bank is not able to reach me for the fulfilment of any service request / offer or card delivery or complaint resolution, it will
contact the alternate contact person (as per the details provided by me to the Bank in this form) for the same. The Bank at its discretion may entertain
enquiries from the alternate contact person about the card account and share transaction details with him / her post verifying his / her identity in order to
ensure that I enjoy uninterrupted services on my Card. I authorise the Bank to deal with the alternate contact person and acknowledge that the Bank will
not be in breach of secrecy obligations by disclosing Card account information to such alternate contact person. I am aware that the card upgrade would
be effective within 30 days from the date of receipt of consent by HDFC Bank and HDFC Bank reserves the right to withdraw the upgrade offer if the Card
account is not in regular status at the time of upgrade. I understand that HDFC Bank cannot be held responsible for financial / non-financial obligation
caused due to withdrawal of offer or non-implementation of Card Upgrade and Limit Enhancement / Card Upgrade offers are subject to me fulfilling all the
necessary and sufficient criteria as decided by the Bank at the time of offer fulfilment. I agree that HDFC Bank reserves the right to withdraw the Limit
Enhancement and Card Upgrade offered, at any time as deemed right by the Bank. I take the responsibility to communicate the new Card number to all third
parties who have been given standing instructions for direct debit on my existing Card number for any service availed thereon. I also take the responsibility
to destroy the existing Card along with existing Add-on / Supplementary Card(s) (if any) on receipt of the upgraded Card. The proposed Limit Enhancement
and Card Upgrade programme would be governed by the existing Terms and Conditions applicable for the current Card held by me and the Terms and
Conditions mentioned here are over and above the existing Terms and Conditions.

Nothing contained above shall prejudice or affect the Terms and Conditions of the Cardmember Agreement. The words and expressions used herein shall have
the same meaning as in the Cardmember Agreement. The terms of this scheme shall be in addition to and not in derogation of the terms contained in the
Cardmember Agreement. Any dispute arising out of this programme shall be subject to the exclusive jurisdiction of competent courts in Chennai irrespective
of whether courts in other areas have concurrent or similar jurisdiction. The existence of a dispute, if any, shall not constitute a claim against HDFC Bank Cards
Division. Credit Card once upgraded, cannot be re-instated to the old Credit Card number. Once upgraded, transactions done on the old Credit Card would be
reflected in the new Card statement. On upgrade, the old Credit Card would be valid only for a period of 30 days from the date of upgrade.
