Week 1 - 9/6 - Lead RN - You provided appropriate leadership for your team. Keep your team well-organized and on track. MCL
Week 2 - 9/20 Bre, you did a great job vision screening new students and third graders at Cub Run ES and vision and audio screening 270, 7th graders at
Beverly Manor MS. You demonstrated learning opportunities in both the cognitive and psychomotor domains. You verbalized in your well child journal
that you were surprised at how challenging it was to establish therapeutic communication with some of the third graders due to developmental delays or
English as a second language. You were able to adjust your approach by engaging the children in the vision screening using their hands to make the “Es”
and making it a game with magic glasses to gain their cooperation or help calm them if fearful. You achieved confidence in the ability to accurately vision
screen and you learned how to correctly use the audiometer to check the 7th grader’s hearing. Very good insights regarding how you changed your
approach to make the screenings more effective for the children. You worked very hard. Nice job. Dr. Houlahan

Week 3 - 9/13 - Communication RN - Obj 1 complete and thorough documentation of focused assessment. Your SBAR is coming along. Continue to work

Week 4 10/4/17 Bre, your team did a wonderful job with your interactive presentation for the health fair for 5th graders at Daniel Morgan Middle
School. Your chosen activity of Pedestrian and Halloween safety was creative, engaging and educational. The students thoroughly enjoyed their ability to
learn about pedestrian safety techniques and your demonstration of the “glow in the dark safety tape” clearly showed the importance of being seen to
prevent accidents. You demonstrated clinical competence, critical thinking and prioritization of care in your choice of topic. You therapeutically
communicated at an appropriate developmental level with the 5th grade students. Well done. Dr. Houlahan
Week 5 - 9/27 - Assessment RN - Performed a focused pediatric assessment on a child experiencing dehydration. Appropriately provided a bucket close
at hand to the patient. Identified special needs of children by providing music as a distraction. You and your team all used age appropriate terminology
when talking to the patient. MCL
Week 6 - 10/11 - Bre in your role today as the medication RN you accomplished the sim objective of calculating and administering medications. There
was a delay in administering the medication as discussed in debriefing. You also made sure to correctly document the medication administration in

Instructor comments: Bre, it was a pleasure working with you during your well-child clinical this semester. You consistently demonstrated
professionalism, enthusiasm and appropriate engagement to provide well-child care in the school setting as well as health promotion at the
health fair. Professor Liesen has noted that you exceeded in several areas over the semester: particularly the areas of critical thinking,
prioritization and organization of care, professionalism and leadership, therapeutic communication and teamwork. Well done. Overall you met
and exceeded the required expectations for safe and effective nursing practice this semester. One point of consideration and always an area
for improvement: to remain calm and focused when your patient is compromised and to be sure to communicate effectively with your team
and provider using SBAR. Professor Liesen has suggested in your formative feedback that you continue to work on this skill set. It appears
from your formative feedback from Professor Liesen that you have performed well in the roles of lead and medication nurse and that you have
demonstrated proficiency in completing a focused pediatric assessment. I would encourage you as I am all of the third semester students to
tap into your resources with the skills lab team prior to next semester when you begin your practicum hours. They are happy to meet with you
to support you in your remediation of any skills - especially IV fluids as they are an important element of nursing practice. It takes lots of
experience and opportunities to maintain best practice when under stressful situations with decompensating patients. These skills will
continue to develop as you grow in confidence as a new practicing nurse. Please let me know if you have any questions or concerns. Well

All Precious Time requirements completed. An average on all written clinical assignments (including Precious Time and SimChart, but
excluding MCAST) must equal 70% or higher: This Practicum Evaluation Tool (PET) reflects a passing grade contingent upon successful completion of all
Precious Time clinical hour and paperwork requirements. The faculty will update the PET once all the requirements have been made. If all requirements
are not met by the last day of classes, the student may receive an incomplete or a clinical course failure dependent on the nature of the circumstances of
not meeting course requirements. Faculty will advise the student if the requirements are not fulfilled within the designated timeframe.
