PERSONAL HISTORY (ANAMNESIS)

PRENATAL

Ann the mother of L.A.ride said that she carried herself well and didn’t
experience any bumps, falls and other accidents during pregnancy. She had prenatal
check-ups every month. She never experienced any sickness during her pregnancy to
L.A.ride. She didn’t have any problems experienced during her pregnancy with
L.A.ride and according to her she had normal pregnancy. She is very careful regarding
to her pregnancy. Ann took vitamins and supplements every day and eats nutritious
foods during her pregnancy. But she ingested of unapproved herbal meds.

INFANCY

According to Ann, she experienced a complete nine months of pregnancy. On

June 24, 1973 in Caraga, Ann gave birth to L.A.ride in the hospital through normal
delivery without difficulty and no instrumental sequels. Attended by a doctor.

TODDLER

L.A.ride is breastfed for 3 years after birth. The feeding pattern during infancy
is normal. According to his mother she cuddles and hugs him always when she
breastfeeds him and he showed positive nocturnal enuresis. L.A.ride completed the
immunization.

He was taken care by a “Yaya” and sometimes by his Lola because both of her
parents are teachers and they could took care him during weekends and night time.

His first tooth came out when he was 1 year old. He started talking at age 1 and
also walking at the same age. The toilet training started when he was 2 years old and it
was mostly done by the mother and she was not strict in it. He does it independently
but with a mother’s guidance.

According to Ann, L.A.ride thumb sucks from 2 years old to 5 years old.

PRESCHOOL

On preschool, he was noted to be a friendly kid and negative for sibling rivalry.
Patient’s mother verbalized that the patient experienced being scolded by his father
without any reason and witnessed as his father crashed his car toy which he loves the
most.

SCHOOL AGE

He started schooling at the age of 6 years old. He does not skip classes and his
performance in school is fair. He doesn’t have many friends, he is not talkative and he
is shy. His favorite subject is Sciences. He was a consistent honor student, either a 2nd
or 3rd honor.

ADOLESCENCE

During adolescence, he was still a shy person but he still have friends. Patient
was used to play billiard and Brazilian chess with his friends. And known not to have a
girlfriend at that time.

YOUNG ADULT TO MIDDLE ADULT

Intellectual History

He studied well and focus to his subjects that’s why his grades were fair. His
grades were just enough to pass the subject. He graduated the degree of Bachelor of
Science in Mechanical Engineering on March 1994 at University of Mindanao in Davao
and took the board exam but was not able to pass. He went to Taiwan and worked as a
factory worker for almost three (3) years but after being involved in an incident on year
2000 he went back to the Philippines. The patient had head trauma due to alleged
mauling in Taiwan.

Social History

The client was an active member of his Religious affiliation. He was dedicated
and goes along with his colleagues religiously and acts accordingly. He’s fond of
dealing with his co-members. Later part of his life when he went to Taiwan to work he
experienced head trauma due to alleged mauling in Taiwan. When he went back to
Philippines he became socially withdrawn, suspicious and later became hostile and has
disorganized behavior.
Psychosocial History

He got circumcised at the age of 10 and he started to become aware about sex
is when he was 15 years old and at that age he also started to masturbate according to
his mother. The age when his voiced lowered in pitch is when he is 18 years old.

L.A.ride is not married. However, he did have relationships with the opposite
sex. When L.A.ride was 20 years old, he had his first girlfriend. His mother verbalized
that L.A.ride doesn’t share his love life to her.
 ANAMNESIS

Apparent understanding of present illness of patient:

Si L.A.ride wa nakahuman sa iyang kontrata nga (3) tulo ka tuig sa Taiwan,

nagtrabaho lamang siya sa sulod sa (2) duha ka tuig. Human sa (2) duha ka niyang
panarbaho, niuli siya sa Pinas mahitungod sa aksidente nga gikulata siya sa mga bugoy
sa Taiwan ug siya gibunalan sa ulo nga parte. Tungod niana, diha nagsugod ang iyang
sakit sa pangutok ug diha nakita ang mga symptomas sama sa delusion ug hallucination.
Tuig 2010, nakita ang symptomas nga pagsimang sa iyang mga tubag mao nang ako
siyang gipatambalan sa mental hospital ug gitagaan siya ug tambal. Bulan sa Disyembre
tuig 2010 mao ang pag admit nako niya sa rehab mahitungod kay di na siya makatulog
ug tarong sulod sa usa ka bulan ug didto nagsugod ang iyang sakit nga paranoia. Bulan
sa Marso tuig 2013, siya na admit sa makaduha kay nakita nako si L.A.ride nga anaay
kastorya apan wala siyay kauban ug anaa pud toy panahon nga siya nagsuot ra ug brief
sulod sa balay ug gasulat-sulat dayon siya sa mga ding-ding, pultahan, aparador ug uban
pa. Mao ng nakadisisyon ko nga ipaadmit siya sa makausab kai anaa napud siyay plano
nga patyon ang iyang kaugalingon pamaagi sa pag ambak sa tulay sa Davao river.
Tungod sa mga nahitabo kay L.A.ride, siya gasugod na ug pakita ug mga dili angay nga
pamatasan, sama sa pagpanakit sa iyang kaugalingon mao ng giresitahan siya ug tambal
sa doctor apan wala siya nitumar ug tarong niini mao ng mas nisamot ang iyang sakit.
Bulan sa Enero petsa 13 tuig 2018, giadmit siya makausab, gi-diagnose siya sakit nga
schizophrenia. Makita sa iyaha ang pagka-violente, sama sa gibunalan niya ang iyang
igsoon gamit ang bangko, mao ng amo siyang gihiktan ug gitabunan ang iyang dagway
kay gapamaak pud siya sa muduol sa iyaha. (5) lima ka bulan ug (19) desi-noybe ka
adlaw siya nga anaa sa mental hospital ug kaniadtong bulan sa hulyo (19) desi-noybe
tuig sa 2018 siya nakagawas.

The informant was very accommodating and cooperative. She was open in
discussing about L.A.ride’s mental illness. She shows concern on her son’s condition.
She’s willing to answer the questions that we asked.

2.) Name: Mario

Age: 23

Civil Status: Single

Address: Purok 3, Buhangin Davao City

Relationship to patient: Relative

Length of time known to patient: Nine years

Apparent understanding of present illness of patient:

“Kini si L.A.ride, okay man ni siya sauna. 9 years mi nag-uban kay nibalhin man
kog puyo sa ilaha. Sa pagkaila nako ni Lord, elegante gyud kayo ni siya. Sosyalon kayo
ni siya ug iya mga gamit kay puros branded. Ug sauna college pa daw siya, naa ni siya
mga barkada ba nga iya permi nga makauban, makihalobilo siya pero sa iya ra pud ka-
close. Sauna pud gale kay gabinuang siya sa mga babae. Sa iyang pagkabata wala jud
kayo ko kabalo kay wala man mi nagdungan ug dako. Nakagraduate man ni siyag
mechanical engineering pero wala siya nagtake ug board exam kay kahibalo na daw
siya nga dii siya mupasar. Ug mao to ni abroad nalang siya didto sa Taiwan, nagtrabaho
siya didto sa factory. Pila pud siya katuig didto sa Taiwan ug nabunalan siya baseball
bat sa yang ulo ug wala mi nasayod kung unsay hinungdan. Pero nakauli pa man to siya
diri sa amoa ug normal ug nakaatiman pa gale siya sa iyang papa nga naparalized.
Hangtud sa niabot ang panahon nga galisud na siyag katulog, gatigum-tigum siya mga
bato ibutang niya sa daplin sa iyang purtahan kay naa daw gapanghilabot sa iyaha ug

mental illness. He shows concern on his cousin’s condition. He’s willing to answer the
questions being asked.

3.) Name: Marciano

Age: 42

Civil Status: Married

Address: Purok 3, Buhangin Davao City

Relationship to patient: Younger Brother

Length of time known to patient: Since birth

Apparent understanding of present illness of patient:

Kana si L.A.ride nagsugod gyud na sukad adtong niadto iya ug Taiwan. Nag-
una na siya sa akoa pag adto dayon ang iyang kontrata kay tulo ka tuig unya wa siya
kahuman kay tungod sa nahitabo nga gikulata siya gamit ang kahoy sa mga bugoy nga
nagmotor-motor daplin sa dalan. Miuli siya sa davao human sa nahitabo dayon pag uli
nako gikan sa Taiwan okay pa man siya to dayon pipila ka bulan nangadto mi ug Manila
aron mangita ug trabaho pabalik Taiwan apan wala mi kakita ug trabaho mao ng niuli
ko ug balik sa davao pero si L.A.ride nagpabilin sa Manila sulod sa tulo ka bulan. Ang

condition via mobile because during our home visitation he worked that time. He shows
concern on his brother’s condition.

Age: 39

Civil Status: Married

Address: Purok 3, Buhangin Davao City

Relationship to patient: Neighbor

Length of time known to patient: Twelve Years

Apparent understanding of present illness of patient:

Kana man si L.A.ride kauban pud na sila sa akong bana nga nag adto sa Taiwan
aron manarbaho. Pipila ka bulan niana akong bana nga naggamit na siya ug drugs diadto
unya wala na nahitabo ang gi ingon nila nga gibunalan silbi ila ranang gipagawas nga
ing ana ang nahitabo kay L.A.ride sa Taiwan. Ni uli na siya pero sa Manila nagstay
didto kay gapadayon gihapon daw siya ug gamit ug drugs. Ug adtong nahurot-hurot na
ang iyang kwarta miuli na siya diri sa Davao ug didto na nakita nga nabag-o siya nga
lahi na iyang inistoryahan, lahi na iyang nilihokan ug ga-sige nalang siya ug puyo sa
