This document contains a sample garbage record book form as required by MARPOL Annex V for recording shipboard garbage operations and disposal. [1] The form includes sections to record the ship's particulars, categories of garbage, operational discharges within or outside special areas, and any exceptional discharges or losses. [2] For each discharge or disposal operation, records must include the date, time, position of the ship, estimated amount and category of garbage, and remarks including certification. [3] The master must sign the record book to certify its contents.
