a) Describe three causes of concrete formwork failure:

b) If temperature drops during construction operations, what should you do (in terms of the
concrete placement) in order to prevent a buildup of lateral pressure overloading the forms.

stresses for temporary structures or for temporary loads on permanent structures. What is
the adjustment factor that should be applied to the bending stress if the forms will be used
only once (single-use form)?

b) Why do they commonly use double-member wale in formwork design?
In order to avoid drilling of timbers, they commonly use double-member wale.
c) What is the definition of slenderness ratio and what is the maximum slenderness ratio
allowed for wood members?
Slenderness ratio is the ratio of unsupported length (l) of a member to the width (d) of the
face of the member under consideration. For wood members, l/d cannot exceed 50.
d) What is the slenderness ratio of the column shown below (2 directions)? Is it acceptable?

The controlling slenderness ratio would be 52 (over 50) and column would not be
