Question 1:
a) Describe three causes of concrete formwork failure:
1. Improper Stripping and Shore Removal
2. Inadequate Bracing
3. Vibration
4. Unstable Soil Under Mudsills, Shoring Not Plumb
5. Inadequate Control of Concrete Placement

b) If temperature drops during construction operations, what should you do (in terms of the
concrete placement) in order to prevent a buildup of lateral pressure overloading the forms.
The rate of concrete placement has to slow down.

c) Describe the following symbols: (5 Points)

a) S2E: Lumber which has been surfaced on two edges


b) S4S: Lumber which has been surfaced on all four sides

Question 2:
Draw the pressure diagram for a wall form 15 feet tall in which concrete will be placed at a
rate R = 10 feet per hour when the temperature is 60oF. Draw the diagram if the wall were
only 6 feet tall.

If the placement rate (R) is from 7 to 10 feet per hour, the least
of the following values apply; yet never less than 600 psf:

Question 3:
A contractor uses 6,000-lb form ties spaced at 36 inches on center along the wales. The wales
are also spaced at 30 inches on center. The concrete temperature at the time of placement is
70°F. What is the maximum allowable placement rate (R)?

Wale spacing = 30 in. = 2.5 ft.

Question 4:
a) For form work materials with limited reuse, ACI 347 permits design using allowable
stresses for temporary structures or for temporary loads on permanent structures. What is
the adjustment factor that should be applied to the bending stress if the forms will be used
only once (single-use form)?

b) Why do they commonly use double-member wale in formwork design?
In order to avoid drilling of timbers, they commonly use double-member wale.

c) What is the definition of slenderness ratio and what is the maximum slenderness ratio
allowed for wood members?
Slenderness ratio is the ratio of unsupported length (l) of a member to the width (d) of the
face of the member under consideration. For wood members, l/d cannot exceed 50.
d) What is the slenderness ratio of the column shown below (2 directions)? Is it acceptable?
