for the Rehablitation of FVNHS Campus Stage

Whereas, the flooring of FVNHS Campus Stage is no longer friendly to learners and
does not
met the requirements for Child Friendly School System

Whereas, the FVNHS Campus Stage is the only area to accommodate school based,
district
based and municipal based competition in any Deped Programs, Activites and
Projects (PAPs) implementation.

Whereas, the school is finacially incapacitated to support this project nor the GPTA too.

Now therefore be it resolved, as it is hereby resolved to solicit Hon. Cherry B.

Daraman in the
amount of _________________.

Resolved further that the GPTA Presdient Evelyn P. Clar would personally in
hand this solicitation to the benevolent official.

A RESOLUTION MAKING RURAL BANK OF TANDAG AS THE DEPOSITORY

OF FELISBERTO VERANO NATIONAL HIGH SCHOOL (fvnhs) GENERAL
PARENT-TEACHERS ASSOCIATION (GPTA) AND AUTHORIZING
GENEVIEVE P. DAMARILLO, SCHOOL PRINCIPAL, EVELYN P. CLAR, GPTA
PRESIDENT AND JANETH M. CABITAY, GPTA TREASURER AS THE
OFFICIAL SIGNATORIES IN ANY TRANSACTION OF THE FELISBERTO
VERANO NATIONAL HIGH SCHOOL GPTA WITH RURAL BANK OF TANDAG
(RBT), TANDAG CITY.

WHEREAS, the Genenral Parent Teachers Association and Board Members of

Felisberto Verano National High School of Poblacion, Cortes, Surigao del Sur has
approved tomopen a Savings Account in the rural Bank of Tandag, Tandag City to
safeguard its financial assets;

WHEREAS, the GPTA of the said institution authorized its School Principal,
GPTA President and GPTA Treasurer as the official signatories in any transaction of the
FVNHS GPTA with the Rural Bank of Tandag, Tandag City.

NOW THEREFORE:

BE IT RESOLVED AS IT IS HEREBY RESOLVED to make the Rural Bank of

Tandag, Tandag City as thye official depository of Felisberto Verano National High
School and authorizing the School Principal, Mrs. Genevieve P. Damarillo, Mrs. Evelyn
P. Clar, GPTA President and the GPTA Treasurer Janeth M. Cabitay as the official
signatories in nay transaction of FVNHS GPTA with the Rural Bank of Tandag, Tandag
City.

RESOLVED FURTHER, to furnish copies of this resolution to the management of

the Rural Bank of Tandag, Tandag City for thei information and guidance.

by all present .
