Dear _________________________:

The undersigned is a first year college at Saint Paul University Philippines. I am presently enrolled with
my NSTP 1 subject and one of my major requirements is to render special service to the community.
Since I am taking up Bachelor of Science in Nursing, the service I would like to render is in line with
health. With this, may I ask permission that I may be allowed to check the blood pressure of the people
in our community, particularly adult individuals. This activity is aimed to promote blood pressure
awareness and healthy lifestyle. In 2012, Dr. Dante Morales, the President of the Philippine Society of
Hypertension (PSH) said that 21 percent of Filipino adults are hypertensive. Hypertension or high blood
pressure remains to be a risk factor for a person to suffer a heart attack or stroke or to acquire other
serious illnesses. Prevalence of hypertension in the country is still increasing, thus the idea of promoting
blood pressure awareness to adult individuals is planned.

I hope that this short-term activity will in any way help the people in our community. With this, I
anticipate for your favorable response.
