AFFIDAVIT OF EXPLANATION
I, RISSAN C. MERCADER., of legal age, Filipino, married, with postal
address at Matanao, Davao del Sur, Philippines under legal oath, depose and
say:

That I am the same and identical person who caused the execution of this
instrument.

That I am the natural father of a certain Verla Mae T. Mercader who was
born on September 8, 1994 at Lubo, Poblacion, Zone II, Sta. Cruz, Davao del
Sur;

That at the time when Verla Mae T. Mercader was born, I wasn’t married
yet to her mother GERLIE D. TAYLARAN;

That It was only on November 18, 1995 that I and the child’s mother got
married;

That the birth of the aforesaid child was duly registered at the Office of the
Local Civil Registrar of Digos City;

That, pursuant to Republic Act 9255 (An Act allowing Illegitimate Children
to Use the Surname of their Father), I am hereby authorizing the aforesaid child
to use my surname;

That I am executing this Affidavit for the purpose of allowing the aforesaid
child to use my surname and for civil registry purposes.

IN WITNESS HEREOF, I have hereunto affixed my signature this 13 th day


of July, 2010, Digos City, Philippines.
