This is to certify that I am granting my son/daughter HEIDI D. CAYOBIT permission to


participate in the Practice Teaching Program for a total of Five Hundred and Forty (540) Hours
starting on September 9 until December 13, 2019 at MacArthur National High School
(McNHS) in partial fulfillment of the requirements of Bachelor of Secondary Education
(BSED).

I understand that the son/daughter should strictly observe the rules and regulations of
MacArthur National High School (McNHS) and Abuyog Community College (ACC) in relation
to the Internship Program and to follow all class policies implemented by the Student
Teaching Coordinator in relation to the same.

I am aware that the Student Teaching Coordinator will conduct visitation and monitor the
performance of the student and the Immediate Supervisor will take all the necessary
precautions to keep my son/daughter safe. I am also aware that I can be called should there
be any violations that my son/daughter has committed during the duration of the internship
program.

In the event of an emergency, I hereby give permission to the staff of MacArthur National
High School (McNHS) to secure proper treatment of my son/daughter.

I have likewise read the Informational Letter and Parental Consent and is fully agreeable with
all the provisions stated thereon.

INTERNSHIP INFORMATIONAL LETTER AND PARENTAL CONSENT FORM FOR


STUDENT TEACHING PROGRAM

Abuyog Community College, as part of the Teacher Education Curriculum, offers a Practice
Teaching Program that offers the opportunity for students a first-hand experience in their
chosen specialization. Practice Teachers work with a professional community to gain
experience and insight about the realization and demands of the Teaching Profession.

Participation in this program requires the student to work at the Cooperating School and
will be required to:

a. Report to the internship site and complete at the 540 hours required internship
schedule
b. Provide his/her own transportation to and from the internship site
c. Submit a notarized Parental Consent and Internship Contract/Agreement
d. Submit an Internship Plan and Internship Portfolio
e. Must have a Medical Clearance and Insurance Coverage

The student intern must be dependable, responsible, punctual and exhibit a high standard
of professionalism and decorum. Success in the program will depend upon their
commitment as well as parent’s support.

In the event that the student fails to comply with the organizational and program
standards, policies and regulations, the students may be dismissed from the program
without receiving the course credit.

The Parent/Guardian must indicate approval of his/her son or daughter to participate in


the internship program by written consent (separate form).

If there are any questions or concerns regarding the internship program, please call or
see Sheila V. Posas at Abuyog Community College. Reasonable accommodations will be
provided to persons with disabilities or pregnant women if requested.
