    First Semester AY 2017-2018

    Dear Dr. Torres:

    As parents/guardians of __ (name of student)____, taking (HAT/TAP), do hereby give our consent for him/her to have
    his/her Internship Program ahead of the academic calendar of the Far Eastern University - Institute of Tourism and Hotel
    Management, which is part of the curriculum in their chosen discipline in BS Tourism/HRM. We understand that the
    said course in HAT/TAP will require 700/500 internship hours that will require him/her to finish the entire duration on
    the deadline set by the University.

    In this regard, we assure your office that my son/daughter will comply with the following provisions:

     complete the required number of hours based on his/her program,


     enroll during the 1st Semester of AY 2017-2018 the said internship course,
     credit the acquired grade only during the 1st Semester of AY 2017-2018.

    If my son/daughter failed to enroll this 1st Semester AY 2017-2018, I understand to my full knowledge that his/her credit
    hours, grades and all other requirements will not be honored during the 2nd Semester of AY 2017-2018 or any latter time
    thereafter.

    We (I) do hereby release, forever discharge and agree to hold harmless the school, and its administration with the rest
    of its faculty and staff from any and all liability, claims or demands for personal injury, sickness, or death, as well as
    property damage and expenses, of any nature whatsoever which may be incurred by our son/daughter that occur
    during, inside and outside this early internship program.

    This letter is being issued in compliance with the school policy and regulation for students who will complete their
    remaining number of hours for their internship program beyond the specific deadline given by the University in
    accomplishing all the requirements prior to the encoding of grades.
