Following email is self explanatory that is received from Independent Auditor hired by World Bank. Please make a list
for all document available with all contractors. The Auditor shall require following listed documents for review.
Also tentative schedule of OHS auditor is given as
19-20 December 2019 Document review.
21 December 2019 Field visit.

Dear All,
apart from the 14th October workshop deliverables we would like to see the status from each of the contractors the
following

  *   What standards are being used for the various elements for safe operations at the site reference OHS Plan
requirements (Ahmed has already shared a sample)
  *   The Emergency Response procedures
  *   OHS operational Procedure

     *   such as working at heights,


     *   scaffolding,
     *   hygiene standards at accommodations,
     *   Electrical safety,
     *   selection and use of PPE,
     *   Masonry construction,
     *   machinery protection, and so forth
     *   Hazardous energies control, lock out tag out
     *   Hazardous materials (HAZMAT)
     *   Concrete and Masonry construction Safety
     *   Silica and Asbestos safety
     *   Welding and Cutting
     *   Respiratory protection
     *   Hearing protection
  *   Table with a list of Safety procedures and

  *   Incident Accident statistics

  *   Standards being used for construction.


  *   OHS regulatory being used'
  *   Risk Assessment / Job Hazard analysis / hazards communication
     *   List of Works Method Statements and when they were reviewed
     *   List of corresponding JHA
  *   What standards were proposed in the project CEAP & ESMP
  *   List of OHS specifications for contractors included into the Bidding documents
  *   Accountability matrix (describing OHS responsibilities at all levels of the organization, including high
management, low management, leaders, engineers, foreman and workers) i.e. what is in the Job Descriptions
  *   Training matrix requirements for each contractor,
  *   safety standard requirements,
  *   Golden rules (safety rules that cannot be broken without penalty),
  *   Disciplinary procedure,
  *   Prevention programs
     *   hearing protection,
     *   respiratory protection,
     *   ergonomic prevention etc...
  *   Permit required system for hazardous jobs
     *   working in confined spaces,
     *   working at heights,
     *   hot work,
     *   excavations
     *   electrical permits
     *   scaffolding etc
  *   Selection and use of Personal Protection,
  *   selection and management of sub-contractors,
  *   Workplace monitoring, compliance monitoring,
  *   Health monitoring programs, treatment of deviations and non-compliances,
  *   OHS indicators and reporting matrix
  *   Based on the Risk assessments what are the high-risk activities listed for each contract
     *   going on the ground (meaning there is an imminent threat to life,
     *   like working in confined spaces without proper ventilation,
     *   training or means for rescue; or working at heights without proper protection)

  *   Review of OHS management system,


  *   all existing safety procedures vis-a-vis with local regulatory framework EHS guidelines
  *   Please provide a list of all existing safety procedures with an analysis of the content compared with the
hazards/risks from the project).
Look forward to review these for all contractors and also have the base documents from the "Engineer"

We will start work on 19th early as we have a long agenda to cover
