Covidien introduced the Puritan Bennett™ 840 ventilator in 1998, and since then it has been a trusted ,
reliable product used by customers worldwide . With the launch of the new Puritan Bennett™ 980
ventilator in 2014 , Covidien continues to advance the high standards of the Puritan Bennett™ brand .
At Covidien , we are committed to the highest level of customer service. For that reason , we are providing
this notice of end of service and support fo r certain Pu ritan Bennett 840 ventilators . The decision to end
service and support was made because of the difficulty procuring and maintaining older electronic
components and because many of the devices currently in use have reached or are nearing the end of
their useful lives.
Over the years we have made enhancements to the Puritan Bennett 840 ventilator, including the 2001
introduction of the 10.4 inch graphical user interface (GUI ) to replace the 9.4 inch display. The 9.4 inch
version has been out of production for more than 12 years , and it has become increasingly chal lenging to
maintain parts . Consequently, we will be discontinuing the sale of 9.4 inch GUI spare parts on March 31 ,
2015 .
Puritan Bennett 840 ventilators with the 9.4 inch GUI were manufactured between 1998 and 2002 . These
units are between 12 and 16 years old and are considered beyond their useful lives. We strong ly
recommend you consider the available options to transition out of this equipment. If you own Puritan
Bennett 840 ventilators that were manufactured during the above-mentioned dates that were upgraded to
the 10.4 inch GUI , we will continue to service and offer spare parts through December 31 , 2016 . To
determine whethe r you have a 9.4 or 10.4 inch GUI , please refer to Attachment I.
We are also approaching end of service for other Puritan Bennett 840 ventilators. Puritan Bennett 840
ventilators manufactured between 2003 and 2006 will reach their end-of-service lives on December 31 ,
2018 , and Puritan Bennett 840 ventilators manufactured between 2007 and 2008 will reach end of service
on December 31 , 2020 . If your device is subject to an active service contract, the terms of that contract
supersede these dates.
Please refer to the table below for a list of the ventilators described in this notification and their end -of-
service dates . For your convenience , we have attached the list of ventilators our records indicate your
facility has purchased du ring the mentioned timeframe.
Covidien will contin ue to manufacture and sell the Puritan Bennett™ 840 ventilator in some geographic
locations . For ventilators manufactured after the dates referenced in this letter, Covidien intends provide
service and spare parts for a period consistent with the 12 year or 50 ,000-hour useful life expectation for
the product Notice of the end-of-service dates associated with ventilators sold after the above-mentioned
manufacturing periods will be communicated soon .
Please note that Covidien recently introduced the Puritan Bennett 980 ventilator for sale in the United
States. The Puritan Bennett 980 ventilator helps enable patients to breathe more naturalll through some
of the most innovative breath delivery technology available. To encourage Puritan Bennett 840 ventilator
customers to transition to the new Puritan Bennett 980 ventilator, we are offering various programs that
provide trade-in va lue for your Puritan Bennett 840 ventilator toward the purchase of a new Pu ritan
Bennett 980 ventilator.

To identify Puritan Bennett™ 840 ventilators with a 9.4 inch graphical user interface (GUI) assembly, look
at the back of the GUI assembly. If it has one RS-232 port it is a 9.4 inch GUI assembly; if it has three
RS-232 ports, it is a 10.4 inch GUI assembly.
Note: Some ventilators have been upgraded to the 10. 4 inch GUI assembly in a field upgrade. If your
ventilator's seria l number falls within the serial number range mentioned above, but has a 10.4 inch GUI
assembly, the end-of-service date is December 31 , 2016.
