    AFFIDAVIT OF SUPPLEMENTAL REPORT

    I, JEMAR MARTICIO PARAS, of legal age, Filipino, and a resident of Cebu


    City, after having been duly sworn to in accordance with law hereby depose and
    state:

    1. That my name is JEMAR MARTICIO PARAS, the document owner of


    the herein attached Local Civil Registrar Copy of Certificate of Birth and made
    an integral part of this affidavit;

    2. That I was born on February 1, 1991;

    3. That the Registry Number of the abovementioned document is 91-


    110;

    4. That the entry omitted during the time of registration is my sex which
    is Male;

    5. That such omission was due to the punching of holes in the said
    entry;

    6. That I am executing this affidavit to attest to the veracity of the


    foregoing facts and for whatever legal purpose this may serve.

    IN WITNESS WHEREOF, I have hereunto set my hand this 11 th day of


    January 2019 at Mandaue City, Cebu.
