Mistake

Mistake reports were filed for three incidents: 1) Welding was done near column E10 without first cleaning the area, 2) Welding spatter fell near stored bags without causing smoke at the time, and 3) After finishing work, smoke was later discovered coming from the bags, caused by welding spatter that had fallen earlier without cleaning. The report concludes that not cleaning the work area before and after could have led to a big mistake, and care will be taken to clean areas in the future.

MISTAKE REPORT

Point no: - 01
We started work near column number E10 in D1 block but did not clean up
Before the hot work activity.
Point no: - 02
Pipe fabrication was going on at the top and some welding machine spatter was
falling at bottom zero level area. There were some sacks and bags kept below
Which the spatter were going under the bags. That time there had no smoke.

Point no: - 03
When we finished the work there, after that we did not clean, we went after that,
There was smoke and the cause of the smoke in the plastic bag was the welding
Machine spatter itself.

This could have been a big mistake because of not cleaning the area before work,
we will take care to clean the area before work and even after work .
