LETTER

OF COMMITMENT
(For Players)

I fully recognize that as a selected player of a team participating in the NBTC League, I am representing the
league, its sponsor(s) and my school. I therefore agree to adhere to the core values of the program: love of
country, passion for the game, integrity, leadership and commitment to excellence and shall always uphold
its objectives: Developing skills, Building character and Raising leaders.

I commit to obey all the rules and regulations of the NBTC and observe the following code of conduct:
• To respect the authority of the Program Director, Coaches and other officials and members of the
NBTC League at all times and abide by all their decisions.
• To be present in all training sessions, functions and games, and report on time.
• To wear the prescribed uniform every training session, function and game.
• To handle all equipments with utmost care.
• To keep all facilities used clean and orderly at all times.
• To conduct myself properly and in all sportsmanly manner, on and off the court.
• Thus, I will not swear, curse, bully nor use any obscene/offensive language or gesture, and will
respect my fellow trainees.
• Not to smoke, drink liquor or take any illegal, prohibited or banned substances.
• To always play by the rules and never question or argue with any official during training and
games.

I fully understand the importance of compliance with the rules and regulations of the NBTC League and the
foregoing code of conduct. I therefore submit to the right and authority of the NBTC League to remove me
from the program should I violate any of its rules and regulations or any of the above-enumerated code of
conduct.

WAIVER
(For Parents)


This is to confirm that I am allowing my son/ward to participate in the SM NBTC League Local City

Qualifiers to be held in our city/province. I understand and acknowledge the risks involved in his

participation in the games supervised by the NBTC League, including the travel from our place to the

tournament and vice versa. In case of an injury, I give permission to the organizers to seek the appropriate

medical attention for my son/ward. I therefore release the NBTC, its organizers, officers, employees and

sponsors from any liability resulting from any injury, loss or damage that my son/ward may suffer while

traveling to and from the venue and during the competition.

This is also to confirm that my son/ward is eligible to play and I assume full responsibility for all the

documents submitted for which I can be criminally liable in case any documents turn out to be false or

tampered.
