    I, ___________________, of legal age, Filipinos and resident of
    ______________________________, have this day named, constituted and appointed my
    son, ___________________________of legal age and resident __________ San Carlos
    City, Negros Occidental, Philippines, to be my true and lawful Attorney-in-Fact, to do and
    perform in my behalf, the following acts and deeds:
    1. To sign and file a Criminal Case against _________________________
    on my own behalf;
    2. To represent and appear before the Prosecutor’s Office and the Courts in
    all stages of the proceedings;
    3. To enter into any amicable settlement if the circumstances warrant,
