Branch Manager of Pest Science Corporation with business address
at ___________________ City duly authorized by virtue of a
Secretary’s Certificate dated September 6, 2019 and entered into the
notarial book of _________________ with Doc. No. ___; Page No.
____, Book No. ____, Series of _____, after having been sworn
according to law, hereby depose and state: THAT---

1. I am the Authorized representative of the Corporation being


the Branch Manager and by virtue of a Secretary’s
Certificate issued to that effect;

2. That last ______________ we decided to cease business


operations due to reasons that we do not have any available
space or office where we could operate our usual business
transactions;

3. By reason of the foregoing, from the months of January 2017


up to the present, there has been no income from our
business by reason of the cessation of our business
operations;

4. This affidavit is being executed as a supporting document for


the purpose of applying for a business permit as the
Corporation intends to continue with its business operations,
and for such other legal purpose that this may serve.
