Identification Requirements
In order to meet the requirements of registration the CLPNA requires two forms of government issued ID. CLPNA
accepts passport, birth certificate, Permanent Resident card, citizenship certificate, and driver’s license. Please note
that at least one has to be photo identification and preferably both will be color copies. Do not fax identification.

Jurisprudence Exam
Applicants are required to successfully complete the CLPNA Jurisprudence Exam prior to the application being
assessed. The exam reflects the legislation, standards and ethics required by the CLPNA to practice as an LPN in
Alberta. Each province has its own legislation; therefore, you will need to complete the Alberta Jurisprudence Exam in
order to practice here, even if you have completed another province’s jurisprudence exam. There is a study guide for
the exam on the CLPNA website at http://www.clpna.com/members/jurisprudence-examination/. More information
on how to access the Jurisprudence Exam will be sent to you once your application has been received.

Registration fees are not refundable, non-transferable.
Payments are to be made payable to CLPNA and prices are subject to change. (NO personal cheques).
