11. Two pieces of latest 1x1 photo with white background
12. One photocopy of any of the following:
− Birth or Baptismal Certificate and Marriage Contract (for married women only)
− Birth, Baptismal or Marriage Certificate (for male)
− Passport
REQUIRED DOCUMENT/S − Voter’s ID, SSS, GSIS, Driver’s License, Postal ID, Police Clearance, and NBI,
(not expired) with address in Caloocan City
− Billing Address in Caloocan City: MERALCO, MAYNILAD, or Any Statement of
Account (SOA)
For lost Senior Citizen’s Identification Card:
1. Affidavit of Lost
2. One piece of 1x1 latest photo with white background
3. Any government-issued Identification Card or any billing addressed in Caloocan City
Senior Citizens with No Record at requirement b:
1. Birth Certificate of one of the children with the senior citizens’ age
2. Home visitation is required.
For Representative of Senior Citizen:
1. Authorization Letter
REQUIRED DOCUMENT/S
2. Valid Identification Card of representative
3. Whole body picture (3R) of senior citizen together with the representative holding
latest newspaper
Note:
All discrepancies in the above mentioned requirements must be supported by an Affidavit.
