LEARNING FEEDBACK DIARY

I.DAILY ACTIVITIES (12/5/19)

TIME ACTIVITY
6:15 AM Travel to EVRMC
6:45 AM Morning circle was conducted. Endorsement of patents were made from previous shift
to 7AM shift
7:00 AM Start of shift. Received the patient
8:00 AM Taking of 8AM vital signs of each patient in Room 3
9:00 AM Bedside care done to assigned patient, medications were administered
12:00nn Taking of 12nn vital signs of each patient in Room 3
1:00PM Bedside care, other nursing interventions done.
2:00PM Charting on patient’s chart done
3:00 PM Endorsement to incoming nurse on duty

II. SIGNIFICANT LEARNING EXPERIENCE

During the second week, I was able to learn as many things as if it was the first. This
week was crucial since we all needed to acquire a case. As I received my patient, I didn’t
think much of her, however as I read her chart, I discovered that there is more than what
meets the eye, and because of that, I was able to take her situation as my case. I learned
through this that each person- each patient in the ward has a story and that we should not
easily judge them, and instead try to understand them and give our care as aspiring
registered nurses.

III. CHALLENGES

The challenges I faced during this day was mainly because of my patient. During the
start of the shift, she seemed to be tired and her posture was slightly slumped. She told me
that there was still pain in her perineal area so she cannot move as much. However, during I
administered her medications, she was no where to be found. She had her meds taken at
12nn instead of 8am.

IV. FUTURE PLANS

My future plans for the next duties are to incorporate what I have learned these past
few weeks in order to better myself in this field. I especially plan to develop my
communication skills because it is hard for most of the patients to understand bisaya, and
during my talks with the patient she would not understand my questions because I
sometimes revert to talking in bisaya.

II. SIGNIFICANT LEARNING EXPERIENCE

During the second day of our ward duty, I was able to receive a new patient. In the
middle of the shift, the patient had a fever that reached a temperature of 39C. My significant
learning experience was that I was able to care for my patient through the performance of
tepid sponge bath. However, I was only able to instruct the husband of the patient in what to
do since the patient herself wanted her husband to do it for her. I also learned that the cause
for her fever is the accumulation of milk in her breasts.

III. CHALLENGES

The primary challenge I had during the shift is my patient’s fever. She would
constantly have high temperature, and the only thing I was able to do is instruct and help
with the tepid sponge bath, as well as adamant encouragement of the patient to always
breastfeed so that the milk in her breasts would be reduced.

IV. FUTURE PLANS

My future plans for the next ward duty are to give the necessary care to my patient,
especially for patients who exhibit above normal findings, such as my patient during this
shift. I also plan develop my communication skills because it is hard for most of the patients
to understand bisaya.

II. SIGNIFICANT LEARNING OF THE DAY

During this shift, I was able to learn more of the patient who was to be the subject of
m case study. I interviewed her and was able to listen to her story. Moreover, I was able to
perform a physical assessment of her body. I was surprised to have learned a lot and had
enjoyed my time with getting to know the patient. The significant learning of the day is that in
order to form a relationship with the patient that is built on trust and respect, we must always
establish rapport and slowly gain the patient’s trust.

III. CHALLENGES

The challenges I encountered in this shift was during the physical assessment. The
client was embarrassed to show some parts of her body, and I understood her part.
However, because of it, I was not able to acquire an accurate assessment in those areas.

IV. FUTURE PLANS

My future plans for our next duties or the next time I perform a physical assessment
on anyone is that I will make sure to provide them with as much privacy as I can so that they
will be less embarrassed and more likely to cooperate fully with the assessment. I also
especially plan to develop my communication skills because it is hard for most of the
patients to understand bisaya, and this patient was no exception as well.
