1] Work start –up and 1.1 Unauthorized work 1.1.1 Obtain a hot work permit before starting the job
obtaining Work Permit 1.2 Invalid work permit 1.2.1 Get the permit validated for the specific day
1.3 Lack of competency 1.3.1 Supervisor must ensure that all work activities are carried out
by competent personnel
1.4 Lack or poor communication 1.4.1 Conduct toolbox meeting, review the Method statement and
Job Safety Analysis to all team members involved in the work
activities.
1.5 Haste or shortcut 1.5.1 Responsible person will conduct inspection, monitor the work
activities and have to ensure safety standards and procedures
are strictly adhered to.

2] Inspection of worksite, 2.1 Vehicular Movement 2.1.1 Be cautious of vehicular traffic


tools and equipment 2.2 Trailing cables on work path 2.2.1 Tape down cables or reroute them to avoid trips.
2.3 Defective/uncertified tools and equipment. 2.3.1 Ensure that tools and equipment are in good condition.
2.3.2 Use only certified/functional tools and equipment for the task.

3] Commence cutting and 3.1 Rotating equipment sparks. 3.1.1 Ensure guards are fitted to all moving parts
grinding 3.1.2 Ensure the proper use of guards
3.2 Damaged grinding disc. 3.2.1 Inspect the grinding discs prior to starting work (ring test)
3.2.2 Ensure that disc is compatible with the grinding machine
3.2.3 Check the RPM of disc and machine
3.3 Electrocution. 3.3.1 Use equipment with double insulation
3.3.2 Ensure there are no exposed portions on electric cord
3.4 Grinding dust 3.4.1 Use proper PPE (mask)
3.5 Noise and vibration 3.5.1 Use only equipment in good condition, wear ear protection
and observe adequate rest break/job rotation.
3.6 Flying chips/eye injuries. 3.6.1 Only experienced personnel to use the equipment
3.6.2 Wear a face shield while grinding
3.6.3 Make sure an eye wash station is available nearby

4] Commence welding 4.1 Fire hazard 4.1.1 Before starting welding make sure the area is clear of all types
of combustible materials.
4.1.1 Provide a fire blanket on site
4.1.3 Make sure fire extinguishers are available on site
4.1.4 Use designated fire watch
4.2 Burns 4.1.5 Carry out regular gas monitoring where necessary
4.2.1 Wear proper PPE for welding i.e. leather jackets, gloves,
welding goggles/welding mask, etc.
4.3 Electrocution 4.3.1 Ensure proper earthing of equipment
4.3.2 Use only certified equipment and certified welders
4.4 Damaged cable
4.4.1 Ensure that welding cables are in good condition.
4.4.2 Use proper cable connector for extension
4.4.3 If cables are crossing the road then ensure proper protection
4.5 Hot welding rod 4.5.1 Ensure that rod holder is in good condition
4.6 Sparks 4.6.1 Make sure spark containment is used such as a welding

4.7 UV rays 4.7.1 The welder and the welder helpers must wear welding goggles
(UV protective)
4.8 Work over water
4.8.1 Personnel to wear work vest and life buoys to be stationed
nearby.
5] Clean-up work site and 5.1 Scattered tools/materials 5.1.1 Carry out housekeeping and ensure that all remnant electrodes
close out of work permit 5.2 Failure to close work permit after task. are removed and materials are stored in a safe place.
5.2.1 Closeout permit properly.
