Old Harbour High School


SIXTH FORM APPLICATION
(GRADE 13)

All Grade 13 students are believed to be the premier students of


the institution. Therefore it is each student’s responsibility to
model the ideals on which the institution stands in order to build
a rich legacy of excellence.

Qualification for Promotion to Grade 13


1. CAPE passes in at least THREE (3) subjects.
2. Accumulation of 80% attendance in Grade 12.
3. Satisfactory conduct and punctuality.

4. The following documents are required


(a) Copies of last two (2) reports.
(b) Copies of CAPE examination results.
(c) One passport size photograph

N.B. A TRANSFER FORM must be completed by all students applying from other schools. The
form is to be signed and stamped by the principal or vice principal of the applicant’s previous
school and submitted along with the Sixth Form Application

OTHER REQUIREMENTS

5. Processing Fee: $1,500.00 (NON- REFUNDABLE). This is payable on submission of the


application form. (PAYMENTS ARE TO BE MADE AT THE BANK OF NOVA SCOTIA
PRODUCTION ACCOUNT NUMBER 3201-91)

6. School Fee: $30,000.00


N.B. Students who DO NOT complete school fee payments by November, 2020 will not be given
documents to register and pay for CAPE examinations.

7. The list of students PROMOTED to Grade 13 will be posted on a date to be announced.

8. REGISTRATION: DATE TO BE ANNOUNCED.

9. OPENING OF SCHOOL: DATE TO BE ANNOUNCED

10. STUDENTS WHO DO NOT ADHERE TO THE UNIFORM GUIDELINES WILL BE SENT
HOME AND WILL NOT RETURN UNTIL THE GUIDELINES ARE MET.

N.B. At the end of the 6th Form Programme, a student may be eligible for an Associate Degree
issued by CXC. STUDENTS MUST DO INTEGRATED MATHEMATICS TO BE ELIGIBLE
FOR THE ASSOCIATE DEGREE. (See Associate Degree guidelines.)

At the CAPE level, students are allowed to pursue a maximum of four (4) subjects. These
include Communication Studies or Caribbean Studies which are COMPULSORY subjects.
Students are required to do both, one in each year of the programme.

1. Students must select a maximum of FOUR (4) subjects from either Band 1, 2 or 3. The
choices MUST include ONE of the compulsory subjects. DO NOT select across Bands.
A student must have the subjects at the CSEC Level in Grades 1 or 2 to be qualified to
pursue them at the CAPE Level.
