    JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS


    RE: ONE AND THE SAME PERSON

    We, MADELYN GUINANAS BUCATOT and MYLENE VALDESWIS, both of


    legal ages, Filipino citizens, and residents of Sitio Canumay, Labogon, Mandaue City,
    Philippines, after having been duly sworn in accordance with law, hereby depose and
    state:

    1. That we personally know LUCIA COLIPAPA ARELLANO, who is our


    personal acquaintance and longtime neighbor at Sitio Canumay, Labogon, Mandaue
    City, Philippines;

    2. That said LUCIA COLIPAPA ARELLANO was born on March 04, 1969
    in Quezon, Bukidnon and the fact of registration of her birth at the proper Local Civil
    Registrar;

    3. That we know for a fact that the person by the name of “LUCIA
    COLIPAPA ARELLANO” as appearing in her Birth Certificate and on her Baptismal
    Certificate and the name “LUCIA COLE COMETA” as appearing in her COMELEC ID,
    and the name “LUCIA COLE ARELLANO” as appearing on the Birth Certificate of her
    child JEROME ARELLANO BONIFACIO, JR refers to one and same person and that her
    true and correct name is LUCIA COLIPAPA ARELLANO;

    4. We are not in any way related to any of the foregoing person within the
    fourth civil degree either by consanguinity or affinity, nor do we have any pecuniary
    interest in this matter whatsoever;

    5. We are executing this affidavit to attest to the truthfulness of the


    foregoing statements and to establish the real status and identity of LEONIDA
    BALABA for all legal intents and purposes it may serve.

    IN WITNESS WHEREOF, I have hereunto affixed our signature this October 18,
    2019 at Cebu City, Philippines.
