plan as required by the Ecological Solid Waste Management Act of 2000;

— Project or activity that is proposed for funding is designed from the 10 year
solid waste management plan;

— Proponent LGUs may avail of the fund support once every three years but not
for same activity or project;

— Proponent LGU is committed to provide counterpart funds as evidenced by a:
