● Scissors for long hair if needed
● Antomicrobial solution and applicators, if needed
● Skin preparation set, which contains a disposable razor, compartmentalized basin for solutions, moistureproof drape to protect the bedding, soap solution,
sponges for applying the soap solution, and cotton-tipped applicators for cleaning areas such as the umbilicus
● Warm water to make the soap solution

BEFORE CLIPPING
1. Drape the client. Expose only the area to be
clipped at one time. You will clip about
15cm (6 in.) at a time.

CLIPPING:
2. Make sure the area is dry.

3. Remove hair with clippers; do not apply


pressure.

4. Move the drape, and repeat steps 2 and 3


until the entire area to be prepared is clipped.
If applying antimicrobial solution, follow
steps 10-11.

WET SHAVING
5. Place the moisture-proof towel under the area
to be prepared is clipped.

6. Lather the skin well with the soap solution.

7. Stretch the skin taut and hold the razor at


about a 45 degree angle to the skin. Shave in
the direction in which the hair grows. Use
short strokes and rinse the razor frequently.
8. Wipe excess hair off the skin with the
sponges.

9. Perform hand washing and observe


appropriate infection control procedures.

CLEANING and DISINFECTING
10. Clean any body crevices, such as the umbilicus,
nails and ear canals, with applicators and
solutions. Dry with swabs.
11. If an antimicrobial solution is used, apply to the
area immediately after it is clipped. Leave it for
the designated time, then dry the area with clean
swabs.
AFTER CLIPPING
12. After clipping, the skin preparation
may need to be checked by the nurse in
charge. Report to the nurse in charge any
abrasions, including those made by the
clippers or razor.
13. Remove the waterproof towel and bath
blanket carefully so as not to spill the clipped
hairs onto the bed.
14. Document the skin preparation on the
client’s chart.
1. Record on the client’s chart those
isolation precautions have been
implemented. Cite the type, time and
client’s response.
2. Evaluate using the following criteria:
a. Necessary equipment readily
available
b. Isolation procedure correctly carried
out
c. Patient left comfortable and safe
d. No nasocomial infections originate
from this patient.
