Subject: Proposal and/or Request for Approval/Conformity to Conduct Survey,


Construct, Install, Maintain, Upgrade, Establish, and Operate Converge
Pure Fiber Optic Facilities and Offer Internet Services

Dear Madam:

Converge ICT Solutions, Inc. (CONVERGE), through MetroWorks ICT Construction Inc., a
major provider of wired telecommunications services nationwide is happy to inform you that
CONVERGE has prioritized bringing closely to Samahan Tungo sa Kaunlaran ng
Kumunidad ng Catmon HOA, a community under your jurisdiction, a more efficient Pure
Fiber Optic Facilities that will offer exceptionally proven fast internet service.

CONVERGE uses state-of-the-art pure fiber optic facilities that will provide reliable internet
connection service, especially in this time of pandemic, to employees who are working from
home, students who are into e-learning, and everyone who engages in internet-based
activities. The “New Normal” of today’s economy primarily depends on online transactions
which would create limitless opportunity for everyone to tap. This initiative is in response to
the national government mandate towards providing every community across the country
equal access to telecommunications services.

Henceforth, for this project CONVERGE has commissioned FASTEL SERVICES INC. to
carry out its network expansion program by providing and/or improving Converge Pure Fiber
Optic Facilities in the country.

Hereunder are CONVERGE proposals for your conformity, to wit:

I. CONVERGE ICTSI RESPONSIBILITIES:

1. CONVERGE shall, at its own expense, conduct technical survey, provide and
install the fiber optic cables and other equipment necessary for the activation of
the broadband data, internet and other value-added service facility which include
installation of new poles and/or using existing poles or if necessary laying of
underground cables, and maintain, upgrade, and operate the said installed
facilities at Samahan Tungo sa Kaunlaran ng Kumunidad ng Catmon HOA. Any
upgrading of such facilities and equipment shall be done by CONVERGE, with
the prior written approval of HOMEOWNER’S ASSOCIATION INC.

2. CONVERGE warrants that all its activities and services are lawful and with
necessary government permits and licenses; its services shall be of
efficient and of high quality and its telecommunications facilities,
maintained and in good condition.

II. STKKC HOA RESPONSIBILITIES:

1. STKKC HOA shall allow CONVERGE to conduct its Service Offering Activities
upon activation of the telecommunications facilities and initial launching free of
charge. The following are the proposed marketing/ service offering activities;

1.1 Booth Set-up at the village clubhouse or any designated area


▪ one straight week (7 days) on the initial launching
▪ 2-3 times a week after the initial launching for such duration agreed by
the parties.
1.2 Actual Demo (same duration with booth set-up)
1.3 Subscribers line installation
1.4 Circulation of Memo (distribution of flyers and letters)
1.5 Posting of CONVERGE Ads at the bulletin board
1.6 Participation in any events within the community
1.7 Maintenance of Facilities

2. Actively participate in CONVERGE’s broadband service offering campaigns within


the community.

below, attesting your conformity thereof and thereby formalizing this document as our official
NOTICE TO PROCEED and MEMORANDUM OF AGREEMENT.
