AO Surgery Reference Lenke Classification

The document describes the Lenke classification system for adolescent idiopathic scoliosis (AIS). It defines 6 main curve types based on the location and structural significance of curves in the proximal thoracic, main thoracic, and thoracolumbar/lumbar regions. Additional modifiers are used to indicate the location of the lumbar apex and degree of thoracic kyphosis. Together, the curve type, lumbar modifier, and thoracic modifier comprise a patient's full Lenke classification.

Structural Criteria (Minor curves) Location of Apex (SRS Definition)

Proximal Thoracic • Side Bending Cobb ≥25° Curve Apex
• T2-T5 Kyphosis ≥+20° Thoracic T2 to T11-12 Disc
Main Thoracic • Side Bending Cobb ≥25° Thoracolumbar T12-L1
• T10-L2 Kyphosis ≥+20°
Lumbar L1-2 Disc to L4
Thoracolumbar/Lumbar • Side Bending Cobb ≥25°
• T10-L2 Kyphosis ≥+20°

Modifiers
Lumbar Spine Center Sacral Vertical Thoracic Sagittal Profile T5-T12
Modifier Line to Lumbar Apex Modifier Cobb Angle

A Between pedicles - (Hypo) < 10°
B Touches apical body(ies) N (Normal) 10° — 40°
C Completely medial + (Hyper) > 40°

Curve Type (1–6) + Lumbar Spine Modifier (A, B, C) + Thoracic Sagittal Modifier (–, N, +) = Curve Classification (e.g. 1B+):
