We are pleased to inform you that your son/daughter, __________________________

of Grade VI – Section A, is one of the candidates for Graduation of School Year 2020-2021.
The elementary journey for our pupils is rapidly drawing to a close, and for
parents/guardians, this is always a special moment. And so, the school makes every effort to
provide a ceremony that will recognize your student’s effort amidst the new modality of
teaching and you as a parents/guardian who with great effort to come to school until at the
end of the retrieval of modules.
With the surge of the positive cases of COVID-19 and the health protocol's
compliance, the Department of Education ordered schools to have a Virtual Graduation
instead of the traditional annual graduation. Furthermore, we want to inform you that the
Virtual Graduation of Batch 2021 with the theme “Strengthening Quality Education Amid
Covid-19 Pandemic” will stream on July 16, 2021 at 2:00 PM on Buenasuerte Elementary

INFORMED CONSENT/PERMISSION FORM FOR

VIRTUAL GRADUATION CEREMONIES
In compliance with Data Privacy Act of 2012 (R.A. 10173), its Implementing Rules and
Regulations, and other applicable laws, Buenasuerte Elementary School is committed to protecting
any information of its data subjects.
As Buenasuerte Elementary School holds the first Virtual Graduation Ceremony, this notice
explains how your student’s personal data will be collected, used, and shared. This pertains to all
students who are candidates for graduation.
THIS FORM MUST BE READ AND SIGNED BY EVERY STUDENT WHO WISHES TO PARTICIPATE AND BY A
PARENT OR GUARDIAN OF A PARTICIPATING STUDENT.
In respect of the virtual graduation “graduation ceremony”, we acknowledge:
1. Consideration of the graduation ceremony must be compliant with DepEd Memorandum 027,S.
2021, Conduct of the K to 12 basic Education Program End-of-School-Year Rites for School Year
2020-2021 in Light of the Covid-19 Public Health Emergency.
2. Consent is voluntary – there is no requirement that the student participate in the graduation
ceremony.
3. Graduation photos taken and sent by the students to their adviser will be the official photo use in
the virtual graduation ceremony.
4. Student’s information like name, section, special strand graduated, parents’ name and address are
expected to be published in the virtual graduation video.
5. We are aware of, and consent to, the audience for the graduation ceremony, and understand there
may be individuals who attend the graduation that the school may have no control over.
6. We are aware that the graduation ceremony may be recorded, distributed and archived by the
school.
7. We are aware of the risks inherent with the graduation ceremony, including relevant or significant
limitations or concerns using online platforms, including but not limited to virtual live streaming in
Facebook.
8. We have received and reviewed the agenda of the graduation ceremony and consent to all of the
activities outlined therein.
9. There are risks and hazards inherent in the very nature of the graduation ceremony and it is our
responsibility to obtain and learn as much as possible about the risks and hazards of a virtual
graduation.
10. The parents/guardians accept and understand that the graduation ceremony is a school sanctioned
event and further the school’s faculty are not responsible for any activities organized or attended to by
students before, during, or after the graduation.
If you choose to participate in the virtual graduation ceremony on July 16, 2021, you must
understand that you bear the responsibility for any bodily injury or privacy breach that might occur.
