    (For the Purpose of Correction on Entry in the Birth Certificate)

    I, CHINITA T. MADRONA, of legal age, Filipino, Married and a resident of Tacal Road, Purok El Rancho,
    Barangay Sta Monica, after being sworn to in accordance with the law hereby depose and state that:

    1. That as of the present time, I am not employed in any capacity at any corporation, company, and
    individual person;

    2. That I have applied for the Correction of Entry in my Birth Certificate, specifically, of Name in the
    Local Registrar Office of Puerto Princesa City, Palawan.

    3. That I am executing this Affidavit of No Employment to attest to the truth of the forgoing and to
    support my application for correction of entry in my birth certificate in the LCRO of Puerto Princesa
    City, Palawan.

    IN WITNESS WHEREOF, I have hereunto set my hand this
