First of all Congratulations on clearing your Exams.

In this
tutorial I will explain how to apply for COC online with series of
Screenshots.
Things that you need for applying COC
1. EXN-45 Form duly signed by MMD Surveyor(s)
2. Valid Medical Certificate by DG Approved Doctor.
3. Valid GMDSS Endorsement (now Called as COC for GMDSS) which is issued by
MMD.
4. DIGITAL Photograph (Scanned copy will not do). Get it from Photographer.
5. Digital copy of your Signature. Refer last page of this tutorial for Signatures.
6. Scanned Copies of self attested Modular Documents.
7. Scanned Copy of Application form which is signed by you.

Before you proceed to fill up you application I strongly recommend to get your
photograph and signature as per MMD specifications.

Scanned copy of your photograph is not accepted by MMD it should be orignal soft copy of
your pic. If you have it as per MMD specifications then its ok otherwise get it clicked from
photographer and tell him you need only soft copy no print show him above specifications. I
also recommend get a copy of your signature from photographer as he can edit it in Photoshop
and make background perfect white and of specified size. Print the last page of this tutorial for
your signature.

COC application can be divided into 3 parts:

1. Get Photograph and Signature, (from Photo Studio)


2. Get your required documents in pdf format (From Internet Café of if you have Scanner
at home)
3. Fill the application and pay fee and upload documents.(From your laptop).

This Number 7 File is should have these modular Cert* in one PDF file only as in
sequence as given below.

Some of you may not need # 9 (RUTC )certificate but if it is showing there then you
need it. Once you have all required things with you then proceed to fill your
application online.

Now Log in to E-Governance and click in this circled link as


shown in Picture below.

Click on Application for COC as shown below in pic

You need to Fill your GMDSS Endorsement Details before you
hit Continue

You will get these details as shown on next page

Once you are done hit Continue button pop up will appear like
this make sure your name is correct and hit OK.

Now page will open with your home address which is already in
filled up.

NEW COC will delivered to this address make sure it is correct.

Hit Continue if all is correct otherwise make the required


changes. Medical Cert details will open now you will need few
details from your medical certificate.

See the next page for more info how to fill these details, it will
vary from doctor to doctor so next page is just a example for
you guys

Hit Continue to pay fee online.

Actually the fee will be paid after application is generated.

Hit OK button to Generate Application Number
Now something like this (on next page) will be displayed
Hit Pay now Button over there as shown in pic.

Once again you have to hit pay now as shown in pic below.
Note: Link for uploading documents is not there.

Once you have paid fee you will be reverted back to your e-
governance page, now click in View Status Link as shown in pic
below.

Now you will note Upload documents link has appeared.

Click on View Submitted Data link in front of COC application

This will open a pop up window, which is your filled application,


take a printout of these pages and sign in given columns. You
need to upload the Scan copy of this application. At this stage
50 percent of your application is complete. Major job remaining
is uploading the documents in correct format. It is very
important part as most of queries by MMD are due to incorrect
format.

If you hit the Upload Now link this will open. Along with some
instructions on photograph and signature requirements which I
believe should have been given before. Anyway now you can
get things done before generating application number.
