The personal information provided on this form is collected under the authority of Section 16 of the Canada Shipping Act, 2001. This information is required for a
Seafarer to obtain a Candidate Document Number (CDN). The information is then used to issue a Canadian Maritime Document and other marine documents in
order to comply with the Act and the Marine Personnel Regulations. The information collected is described in personal information bank entitled Seafarers'
Certificates and Documents (TC PPU 030). Under the provisions of the Privacy Act, individuals have the right of access to, correction of and protection of their

Instruction: Application for a Candidate Document Number (CDN)

Persons who require a Canadian Maritime Document (CMD) or other Transport Canada Marine Safety & Security (TCMSS) document, issued by or on behalf of
TCMSS, under Part 3 of the Canada Shipping Act, 2001, must first obtain a CDN. Persons who provided identity information (names, date of birth or other) that
is not consistent with the information already recorded in the TCMSS databank system, must complete this application.

This form must be presented in person to the closest TCMSS office, together with documents that will clearly identify the candidate.

This form applies to any person who is required to obtain a CMD or other document issued by TCMSS such as, but not limited to:
● Certificate of Competency
● Endorsement
● Certificate of Service
● Provisional Medical Certificate
● Medical Certificate
● Training Certificate
● Application for Direct Examination - assessment
● Application for Examination

A completed application, together with two (2) documents; one as proof of identity and one as proof of citizenship, must be submitted in person to the closest
TCMSS office.
Persons living in remote locations or who cannot visit a TCMSS office may apply by mail to the closest TCMSS office. They must include:
● the completed “Application for a Candidate Document Number (CDN)” form;
and
● a certified copy of both required documents (certified by a Commissioner for Oaths at the candidate's expense).

Documents to confirm the identity of the applicant must be issued by a federal, provincial or municipal authority. The document must be valid and must include the
bearer's name, signature and picture. For example:

● Seafarer's Identity Document
● Canadian Maritime Document
● Permanent Resident Card
● Certificate of Indian Status
● Firearms Licence
● Old Age Security Card
● Federal, Provincial or Municipal Employee Identification Card
● Driver's Licence
● Provincial Health Care Card
● Birth Certificate
● Work, Student, Visitor or Temporary Resident permit issued by the Department of Citizenship and Immigration Canada

Documentation to support the proof of citizenship or permanent residency must be a valid:


● Passport
● Provincial or Territorial Birth Certificate
● Naturalization Certificate
● Registration of Birth Abroad Certificate
● Certificate of Retention
● Citizenship Certificates
● Permanent Resident Card

Documents to confirm the identity of a person must be issued by a federal, provincial or municipal authority. The documents must be valid and must include the
bearer's name, date of birth, signature and the candidate's picture must be part of one of the documents. This application form is available on TCMSS website at
