I. Objectives:

1. The undersigned - He will handle the learners during the remediation class in order to
meet the standard.
2. Principal- a. Approves the Learning Intervention Program Plan
b. Quality Assures the Intervention Plan and IMs
c. Monitors the implementation of the program.
3. Parents of the Learners- Be informed of the program and their cooperation shall be
sought before the start and during the remediation.
IV. Procedures/Mechanics:
One week after the 2nd Quarter Oral Reading Verification, results were analyzed. The low
performance in Mathematics of the Grade V - Dedeles Pupils as reflected in the results of the
Oral Reading Verification Results led to a plan of intervention. Three low performers shall
undergo remediation in Reading, wherein these pupils fail to pass.
After identifying the names of these targeted pupils, instructional design shall then be
developed which includes the development of Instructional Materials (IMs) and Reading
Materials needed in the program.
The Principal shall 1st Quality Assure on the Learning Intervention Program (LIP) Plan
and the IMs and Reading Materials developed before the implementation stage.
Through a conference, parents of the targeted learners shall be informed of the LIP. Their
full cooperation and participation shall also be part of the program and shall be sought during the
remediation class.

IV. Monitoring and Evaluation: The program shall be monitored during the implementation of
the program by the principal and be evaluated after the implementation stage.
