ACR Checking of Form 9

The Schools Division of Mandaue City conducted a district checking of Grade Six student evaluation forms at Subangdaku Elementary School on January 29, 2020. The objectives were to provide reliable learner information, ensure the quality and timeliness of school forms, and reduce discrepancies. Schools from South District Elementary participated. Dr. Ma. Divina M. Flores oriented advisers on guidelines before checking forms 9, 137, and PSA/NSO records. Minor issues arose during checking but were addressed. The activity concluded after verifying forms to help schools prepare documents accurately.

NARRATIVE REPORT
The South District Elementary of Mandaue City Division conducted the
checking of Form 9 or known as Grade Six Student’s Evaluation Form , Form 137-E
and PSA/NSO last January 29, 2020 at Subangdaku Elementary School, Subangdaku
Mandaue City. It was participated in by 6 public schools and 8 private schools of the
district.
Dr. Ma. Divina M. Flores, the Public School District Supervisor of South District
Elementary headed the checking of Form 9, Form 137 and PSA/NSO. Before the
checking proper, Dr. Flores oriented the Grade 6 Advisers on some guidelines in the
checking of the forms. Queries were raised by some advisers regarding the subject
offering of the private schools, the consistency on the arrangement of subjects in the
Form 137 and Form 9, issues of no grades written under the Final Rating, pupils with
no PSA and NSO, the number of years in school and many more.
The group was divided into 4 groups each monitored and supervised by the
school heads. The checking proper of Form 9, Form 137 and PSA/NSO started at 9:00
o’clock morning at the designated area provided by the host school and ended at
around 3:00 o’clock in the afternoon.
Minor problems aroused during the checking process but were then addressed
immediately by the PSDS and School Heads.
Everyone showed a feeling of relief after some sleepless nights preparing the
necessary forms for checking. And everything went well as planned.
