1 Form should be filled-in and signed by the applicant himself/herself.
2 Following documents should be attached with the application form:
a. Attest copy of Final Transcript.
b. Attested copy of CNIC.
c. Affix current photograph.
d. Application in foreign country is to submit the authority letter verified by the concerned consulate with copy of Passport including page on
which exit date from Pakistan is stamped.
e. Signature of the applicant as per CNIC.
Note: Original CNIC/Driving License is to be produced at the time of issuance of Degree.
