CTR: Mr. Witness, do you remember having executed any document in relation to this case?

Respondent’s Witness: Yes, Sir/Mam.

CTR: What document did you execute?

Respondent’s Witness: A judicial affidavit.

CTR: If this judicial affidavit will be shown to you, will you be able to identify it?

Respondent’s Witness: Yes, Sir/Mam.

CTR: Showing you this document marked as Exhibit 1, please go over the affidavit. Is this the affidavit
that you executed?

Respondent’s Witness: Yes, Sir/Mam.

CTR: Please look at the second page of the document. Can you identify whose signature appears above
the name Jose Dela Cruz?

Respondent’s Witness: Yes, Sir/ Mam. That is my signature.

CTR: On the same page, there is a signature on top of Atty. ___________’s name. Can you identify

CTR: Do you affirm the truthfulness and veracity of the said judicial affidavit?

Respondent’s Witness: Yes, Sir/Mam.

CTR: Mr. Witness, I am showing to you the Certificate of Live Birth marked as Exhibit B indicating
petitioner's name as "Ramon Corpus Tan Ko. Is this the same official certificate of live birth in your
records?

Respondent’s Witness: Yes, Sir/mam.

CTR: In Entry No. 7 of the said certificate of live birth, what is the surname of the father?

Respondent’s Witness: Tan Ko, Sir/Mam.

CTR: In the same document particularly Entry No. 12, what the complete name of the mother?
Respondent’s Witness: Trinidad Corpus Tan Ko, Sir/Mam.

CTR: In the same document particularly Entry No. 17, what is the name of the informant?

Respondent’s Witness: T.C. Tan Ko, Sir/Mam.

CTR: Thank you, Mr. Witness. No further questions, your honor.

Purpose: To serve as a direct testimony of the petitioner in relation to the petition for the correction of
entries in the certificate of live birth.

Exhibit 2 - Petition for Correction of Entry filed by the Petitioner

Purpose: To prove that the petitioner’s petition failed to implead and notify the affected or interested
parties as mandated under Rule 108 of the Rules of Court;

Judge: Comments?

Judge: I will resolve the formal offer of evidence, I hereby declare that Exhibits 1 and 2 are now deemed
admitted.
