KNOW ALL MEN BY THESE PRESENTS:
We, ROCHELLE AZURIN and MARK JAYRONE SAPUN, Filipino
citizens, and residents of Simpatuyo, Sta. Teresita, Cagayan, after having sworn to
in accordance with law, hereby depose and say that:
We are among the declared beneficiaries Rodolfo Sapun in his St. Peter
Lifeplan;
That said plan holder died intestate and without a Will on August 31, 2021
at Camalaniugan, Cagayan;
That by reason of his death, we, together with our co-beneficiary Julita
Azurin are entitled to receive Cash Assistance Benefit from St. Peter, Aparri,
Cagayan;
That we have decided to WAIVE, ASSIGN and TRANSFER all our rights
and interest in the aforementioned Cash benefits in favor of our co-beneficiary
Julita Azurin;
NOW THEREFORE, for and in consideration of LOVE, AFFECTION and
LIBERALITY, WE DO HEREBY , WAIVE, ASSIGN, TRANSFER, and
CONVEY by way of Waiver of Rights and Interest unto JULITA AZURIN, her
heirs and assigns, the above-mentioned properties together with all the
improvements found thereon, free from all liens and encumbrances of whatever
nature.
